摘要
目的:研究天水及周边地区吸烟状况与肺功能主要参数的关系。方法:选取2021年11月—2022年11月于天水四零七医院就诊的有吸烟史伴有咳嗽、咳痰、呼吸困难等气道症状患者100例作为研究对象,根据年龄阶段不同分为低龄组与高龄组各50例,低龄组20~40岁,高龄组41~60岁。横断面调查吸烟状况,进行肺功能检查,分析不同年龄阶段、烟龄、日吸烟量与肺功能主要参数的关系。结果:低龄组烟龄、日吸烟量低于高龄组,差异有统计学意义(P=0.000)。两组患者肺功能用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、1秒率(FEV_(1)/FVC)、最高呼气流量(PEF)、用力呼出25%~75%肺活量的呼气流量(MEF_(75)、MEF_(50)、MEF_(25)、MEF_(75/25))参数实测值均低于正常预测值,差异有统计学意义(P<0.001)。低龄组PEF、FEV_(1)、MEF_(75)、MEF_(50)、MEF_(25)、MEF_(75/25)实测值/预计值高于高龄组,差异有统计学意义(P<0.05)。高龄组FVC、PEF、FEV_(1)、FEV_(1)/FVC、MEF_(75)下降例数多于低龄组,差异有统计学意义(P<0.05)。结论:天水及周边地区作为西北欠发达区域,文化层次较低,重工业等对环境污染较少,空气环境较好,对肺功能的损害较为单纯,吸烟为主要的高危因素,烟龄、日吸烟量在不同年龄阶段对肺功能指标损害呈递进关系。
Objective:To study the relationship between smoking status and main parameters of lung function in Tianshui and surrounding areas.Methods:One hundred patients with a history of smoking and airway symptoms such as cough,sputum and dyspnea who were treated in Tianshui 407 Hospital from November 2021 to November 2022 were selected as the research objects.They were divided into the younger group and the older group with fifty patients in each group according to different age stages,the younger group with twenty to forty years old and the older group with forty-one to sixty years old.Cross-sectional investigation of smoking status,lung function examination,to analyze the relationship between different age,smoking age,daily smoking amount and main parameters of lung function.Results:The age and daily smoking amount of the younger group were lower than those of the older group,and the difference was statistically significant(P=0.000).The measured values of forced vital capacity(FVC),forced expiratory volume in the first second(FEV_(1)),rate in the first second(FEV_(1)/FVC),maximum expiratory flow(PEF),and expiratory flow in the forced exhalation of 25%~75%of the vital capacity(MEF_(75),MEF_(50),MEF_(25),MEF_(75/25))in both groups were lower than the normal predicted values,the difference was statistically significant(P<0.001).The measured/predicted values of PEF,FEV_(1),MEF_(75),MEF_(50),MEF_(25) and MEF_(75/25) in the younger group were higher than those in the older group,and the difference was statistically significant(P<0.05).The number of decreased cases of FVC,PEF,FEV_(1),FEV_(1)/FVC and MEF_(75) in the older group was higher than that in the younger group,and the difference was statistically significant(P<0.05).Conclusion:As an underdeveloped region in northwest China,Tianshui and its surrounding areas have a lower level of culture,less environmental pollution from heavy industry and better air environment,and relatively simple damage to lung function.Smoking is the main high-risk factor.Smoking age and daily smo
