摘要
目的:分析无创呼吸机辅助通气序贯高流量吸氧治疗重症急性左心衰竭合并呼吸衰竭的临床疗效。方法:选取2018年4月—2020年6月广西壮族自治区南溪山医院收治的重症急性左心衰竭合并呼吸衰竭患者36例为观察对象,根据随机数字表法分为对照组和试验组,各18例。对照组给予无创正压通气治疗,试验组给予无创呼吸机辅助通气序贯高流量吸氧治疗。比较两组患者住院时间、循环功能、血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、动脉血氧饱和度(SaO_(2))]。结果:试验组ICU住院时间和总住院时间短于对照组,差异有统计学意义(P=0.001)。治疗48 h末,两组患者平均动脉压(MAP)、心率和呼吸频率均降低,差异有统计学意义(P<0.05);试验组MAP、心率、呼吸频率均低于对照组,差异有统计学意义(P=0.001)。治疗48 h末,两组患者PaO_(2)、SaO_(2)水平均升高,PaCO_(2)水平均降低,试验组PaO_(2)和SaO_(2)水平高于对照组,PaCO_(2)水平低于对照组,差异有统计学意义(P<0.05)。结论:无创呼吸机辅助通气序贯高流量吸氧治疗重症急性左心衰竭合并呼吸衰竭的临床疗效显著,可改善患者循环功能及血气指标,缩短治疗进程。
Objective:To analyze the clinical effect of non-invasive ventilator-assisted ventilation followed by high-flow oxygen inhalation in the treatment of severe acute left heart failure complicated with respiratory failure.Methods:Thirty-six patients with severe acute left heart failure combined with respiratory failure admitted to Nanxishan Hospital of Guangxi Zhuang Autonomous Region from April 2018 to June 2020 were selected as the observation objects,and were divided into control group and experimental group according to random number table method,with eighteen cases in each group.The control group was given non-invasive positive pressure ventilation,and the experimental group was given non-invasive ventilator-assisted ventilation followed by high-flow oxygen therapy.The length of hospital stay,circulation function and blood gas indexes(partial oxygen pressure(PaO_(2)),partial carbon dioxide pressure(PaCO_(2))and arterial oxygen saturation(SaO_(2)))were compared between the two groups.Results:The length of ICU stay and total length of stay in the experimental group were shorter than those in the control group,and the difference was statistically significant(P=0.001).At the end of 48 h of treatment,the mean arterial pressure(MAP),heart rate and respiratory rate were decreased in 2 groups,and the differences were statistically significant(P<0.05);MAP,heart rate and respiratory rate of experimental group were lower than control group,the difference was statistically significant(P=0.001).At the end of 48 h of treatment,PaO_(2)and SaO_(2)levels were increased and PaCO_(2)levels were decreased in 2 groups,and PaO_(2)and SaO_(2)levels in experimental group were higher than those in control group,while PaCO_(2)levels were lower than those in control group,the difference was statistically significant(P<0.05).Conclusion:Non-invasive ventilator-assisted ventilation followed by high-flow oxygen inhalation in the treatment of severe acute left heart failure complicated with respiratory failure has significant clinical efficacy
