摘要
目的 评估腹腔镜、开腹手术应用在结直肠癌(CRC)中的效果及对术后肠梗阻的影响。方法 纳入2019年1月至2021年12月的80例CRC患者,全部受试者以随机数字表法分为对照组(纳入40例,行开腹手术)、观察组(纳入40例,行腹腔镜手术),评价组间炎症反应、免疫功能、围手术期参数、并发症状况。结果 (1)术前,组间炎症反应[C反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞计数(WBC)]、免疫功能(CD4^(+)、CD3^(+)、CD8^(+))无差异,P>0.05;术后3d,观察组炎症反应(CRP、IL-6、WBC)、免疫功能(CD4^(+)、CD3^(+)、CD8^(+))更佳,P<0.05;(2)较之对照组,观察组围手术期参数更佳、并发症更少,P<0.05;(3)组间手术时间、淋巴结清扫数目无差异,P>0.05。结论 腹腔镜手术应用在CRC中具有并发症少、炎症反应小、恢复速度快、不易影响免疫功能等优势,值得推广。
Objective To evaluate the effect of laparoscopic and open surgery in colorectal cancer(CRC)and its influence on postoperative intestinal obstruction.Methods A total of 80 CRC patients from January 2019 to December 2021 were enrolled,and all subjects were randomly divided into control group(40 cases were included,underwent open surgery)and observation group(40 cases were included,underwent laparoscopic surgery).The inflammatory reaction,immune function,perioperative parameters and complications between groups were evaluated.Results(1)before operation,there were no differences in inflammatory response[c-reactive protein(CRP),interleukin-6(il-6),white blood cell count(WBC)]and immune function(CD4^(+),CD3^(+),CD8^(+))between groups,P>0.05;Three days after operation,the inflammatory response(CRP,IL-6,WBC)and immune function(CD4^(+),CD3^(+),CD8^(+))of the observation group were better,P<0.05;(2)Compared with the control group,the observation group had better perioperative parameters and fewer complications,P<0.05;(3)There were no differences in operation time and number of lymph nodes dissection between groups,P>0.05.Conclusion The application of laparoscopic surgery in CRC has the advantages of fewer complications,less inflammatory response,faster recovery,and less influence on immune function,which is worthy of promotion.
