摘要
目的观察椎旁神经阻滞联合经颅直流电刺激对带状疱疹后神经痛患者疼痛和情绪的影响。方法选择2022年3-8月于我科住院治疗的72例确诊为带状疱疹后神经痛的患者为研究对象,随机分对照组(A组)和观察组(B组),每组36例。住院期间,两组患者均常规给予营养神经和止痛药物以及超声引导下椎旁神经阻滞治疗;在此基础上,B组患者每天行经颅直流电刺激(transcranial direct current stimulation,tDCS)治疗,A组患者接受假刺激治疗。于治疗前(T0)、治疗结束后1周(T1)、1个月(T2)、2个月(T3)、3个月(T4)评估疼痛数字评分(numerical rating scale,NRS)、状态焦虑量表(state⁃trait anxiety in⁃ventory,STAI⁃S),于T0、T2、T3、T4时评估睡眠质量自评量表(Self⁃Rating Scale of Sleep,SRSS)。评估两组治疗结束后3个月时的疗效。结果与治疗前比较,两组患者治疗结束后各时间点NRS评分、STAI⁃S评分和SRSS评分均明显降低(P<0.05);与A组相比,B组患者治疗后各时间点NRS评分、STAI⁃S评分和SRSS评分均显著降低,差异有统计学意义(P<0.05);治疗结束后3个月,B组疗效更优,差异有统计学意义(P<0.05)。结论椎旁神经阻滞联合经颅直流电刺激较单纯椎旁神经阻滞疗效更佳,能够更有效缓解带状疱疹后神经痛患者疼痛,改善患者情绪及睡眠质量。
Objective To observe the effect of ultrasound⁃guided paraspinal nerve block combined with transcranial direct current stimulation(tDCS)on pain and mood in patients with postherpetic neuralgia.Methods From March 2022 to August 2022,72 patients with PHN were randomly divided into two groups:The control group(group A,n=36)and the observation group(group B,n=36).Both groups were treated with neural nutrients,analgesics and ultrasound⁃guided paraspinal nerve block.The patients in group B received daily transcranial direct current stimulation,while the patients in group A was treated with sham stimulation.Numerical Rating Scale(NRS)and state⁃trait anxiety inventory(STAI⁃S)were assessed at the time of pre⁃treatment(T0),1 week(T1),1 month(T2),2 months(T3)and 3 months(T4)after treatment,self⁃rating scale of sleep(SRSS)was assessed at T0,T2,T3 and T4.The therapeutic effects of the two groups were evaluated 3 months after treatment.Results After treatment,the scores of NRS,STAI⁃S and SRSS in both groups were significantly lower than those before treatment(P<0.05),the scores of NRS,STAI⁃S and SRSS in group B were significantly lower than those in group A(P<0.05).Three months after treatment,the curative effect of group B was better,the difference was sta⁃tistically significant(P<0.05).Conclusions Ultrasound⁃guided paraspinal nerve block combined with transcrani⁃al direct current stimulation is more effective than paraspinal nerve block alone in relieving pain,improving mood and sleep quality in patients with postherpetic neuralgia.
