摘要
目的:观察补阳还五汤结合扶正补土针灸法治疗中风偏瘫的临床疗效。方法:将2020年3月至2022年4月许昌市中心医院收治的94例中风偏瘫患者按照随机数字表法分为对照组和观察组,每组各47例。对照组给予常规西药治疗,观察组在对照组治疗的基础上加用补阳还五汤结合扶正补土针灸法。比较两组患者治疗前后中医证候积分、Bereg平衡量表(berg balance scale,BBS)评分、Fugl-Meyer运动功能评定量表(Fugl-Meyer assessment,FMA)评分、超敏C反应蛋白(hypersensitive C-reactive protein,hs-CRP)、基质金属蛋白酶(matrix metalloproteinase,MMP)-9、血清N末端前脑钠肽(N-terminal B-type natriuretic peptide,NT-proBNP)、同型半胱氨酸(homocysteine,Hcy)及血液流变学指标变化情况。结果:两组患者治疗后中医证候积分低于本组治疗前,且治疗后观察组低于对照组(P<0.05)。两组患者治疗后BBS、FMA评分高于本组治疗前,且治疗后观察组高于对照组(P<0.05)。两组患者治疗后hs-CRP、MMP-9、NT-proBNP、Hcy、血浆黏度、全血黏度、血细胞比容水平低于本组治疗前,且治疗后观察组低于对照组(P<0.05)。结论:补阳还五汤结合扶正补土针灸法治疗中风偏瘫,有利于改善患者的临床症状,提高肢体运动和平衡功能,并能有效降低机体炎症反应,改善血液流变学指标。
Objective:To observe the clinical efficacy of Yang-Supplementing and Five-Returning Decoction combined with acupuncture and moxibustion for reinforcing the healthy and nourishing earth on apoplectic hemiplegia.Methods:A total of 94 stroke hemiplegic patients admitted to Xuchang Central Hospital from March 2020 to April 2022 were randomly divided into the control group and the observation group,with 47 cases in each group.The control group was treated with conventional Western medicine,while the observation group was treated with Yang-Supplementing and Five-Returning Decoction combined with acupuncture and moxibustion for reinforcing the healthy and nourishing earth on the basis of the control group.Then the changes of TCM syndrome scores,berg balance scale(BBS)score,Fugl Meyer motor function assessment(FMA)score,hypersensitive C-reactive protein(hs-CRP),matrix metalloproteinase(MMP)-9,serum N-terminal B-type natriuretic peptide(NT-proBNP),homocysteine(Hcy)and hemorheology indexes of the two groups before and after treatment were compared.Results:After treatment,the scores of TCM syndromes in both groups were lower than those of the same group before treatment,and the observation group′s were lower than those of the control group(P<0.05);The BBS and FMA scores of the two groups were higher than those of the same group before treatment,and the observation group′s were higher than those of the control group(P<0.05);The levels of hs-CRP,MMP-9,NT-proBNP,Hcy,plasma viscosity,whole blood viscosity,and hematocrit in both groups were lower than of the same group before treatment,and the observation group′s were lower than those of the control group(P<0.05).Conclusion:Yang-Supplementing and Five-Returning Decoction combined with acupuncture and moxibustion for reinforcing the healthy and nourishing earth can improve the clinical symptoms of patients with stroke hemiplegia,improve limb movement and balance function,effectively reduce the inflammatory reaction of the body,and improve hemorheology indexes.
