摘要
葛根芩连汤证、白头翁汤证、黄连阿胶汤证、驻车丸证的共同病机为热迫大肠,大肠传导失司,四方均用黄连清热厚肠,以治疗下利不止。葛根芩连汤主要用于阳明气分热较重但未伤阴血,兼有表证;白头翁汤用于血分湿热较重,热毒深入营血之证;黄连阿胶汤用于阴虚邪恋,兼有心烦失眠之证;驻车丸用于寒热错杂兼有湿热,阴血亏虚较重之证。葛根芩连汤证的病位主要在太阳阳明经,病机为气分湿热较重;白头翁汤证病位主要在厥阴肝经,病变涉及血分,病机为肝火、湿热、气滞三者互结;黄连阿胶汤证的病位主要在少阴心肾经,病机以阴虚为本,燥热为标;驻车丸证病位主要在足太阴脾经和足厥阴肝经,病机为久痢伤阴,兼有湿热。葛根芩连汤和白头翁汤均治疗实热证,葛根芩连汤以治疗阳明里热为主,白头翁汤是治热毒痢疾的首选方,黄连阿胶汤治疗阴虚火旺、邪热已入营血分之证,驻车丸为久泻久痢之寒热错杂证专药。临证应辨清病位,分清寒热虚实,灵活选方。
The common pathogenesis of syndrome of Pueraria and Scutellaria and Coptis Decoction,syndrome of Pulsatilla Decoc⁃tion,syndrome of Coptis and Colla Corii Asini Decoction,and syndrome of Parking Pill is heat pressing the large intestine,leading to the large intestine′s failure in conduction.For cold and heat,deficiency and excess,all of them use Huanglian(Rhizoma Copti⁃dis)to clear heat and thicken the intestines,and treat chronic diarrhea.Pueraria and Scutellaria and Coptis Decoction is mainly used for treating severe heat in yang brightness qi aspect without damaging yin blood as well as superficial symptoms.Pulsatilla De⁃coction is mainly used for treating severe dampness and heat in the blood aspect,the heat toxin having penetrated into the blood as⁃pect.Coptis and Colla Corii Asini Decoction is mainly used for treating yin deficiency and lingering pathogens,as well as irritability and insomnia.Parking Pill mainly treats cold and heat in complexity,dampness and heat,and severe yin and blood deficiency.The main disease location of Pueraria and Scutellaria and Coptis Decoction is in the stomach meridian of greater yang brightness,with more severe dampness and heat in the qi aspect.The main disease location of Pulsatilla Decoction is in the liver meridian of rever⁃ting yin,which involves the blood aspect and is combined with liver fire,dampness⁃heat,and qi stagnation.The main disease loca⁃tion of Coptis and Colla Corii Asini Decoction is in the heart and kidney meridians of lesser yin,with yin deficiency as the root,and drynessand heat as the manifestations.The main disease location of Parking Pill is in the spleen meridian of foot greater yin and the liver meridian of foot reverting yin;The disease has a longer course and will inevitably damage the yin.Prolonged diarrhea damages yin,and there is also dampness and heat.Both Pueraria and Scutellaria and Coptis Decoction,and Pulsatilla Decoction can treat ex⁃cess heat syndrome,while the former mainly treats yang brightness internal heat,while the
