摘要
目的探讨鼻咽癌患者临床分期的相关因素。方法选取我院2019年2月至2022年6月收治的72例鼻咽癌患者为研究对象,根据TNM分期标准,确诊为鼻咽癌早期(Ⅰ~Ⅱ期)患者29例,鼻咽癌中晚期(Ⅲ~Ⅳ期)患者43例,分析与鼻咽癌临床分期相关的影响因素,比较不同临床分期患者在基本资料、临床症状、淋巴结转移发生情况、肿瘤生长方式、病理类型、血清学指标方面的差异。结果鼻咽癌Ⅰ~Ⅱ期与Ⅲ~Ⅳ期患者的男女比例、年龄、生长方式、病理类型方面对比差异无显著(P>0.05)。鼻咽癌Ⅰ~Ⅱ期与Ⅲ~Ⅳ期患者的首诊症状、伴随症状、淋巴结转移发生情况对比差异显著(P<0.05)。鼻咽癌Ⅰ~Ⅱ期与Ⅲ~Ⅳ期患者的血清学指标(VCA-IgA、CEA、CYFRA21-1)对比差异有显著性(P<0.05)。经Logistic多因素分析,首诊症状(OR=3.704)、伴随症状(OR=4.229)、淋巴结转移(OR=7.702)、VCA-IgA(OR=7.318)、CEA(OR=7.512)、CYFRA21-1(OR=7.318)是影响鼻咽癌临床分期的相关因素。结论首诊症状、伴随症状、淋巴结转移、VCA-IgA、CEA、CYFRA21-1是影响鼻咽癌临床分期的相关因素。根据不同临床分期患者在临床症状、淋巴结转移发生情况等方面的临床特点,明确临床分期的相关因素,制订有针对性的诊治方案。
Objective To explore the relevant factors of clinical staging in nasopharyngeal carcinoma patients.Methods 72 patients with nasopharyngeal carcinoma admitted to our hospital from February 2019 to June 2022 were selected as the research subjects.According to the TNM staging criteria,29 patients were diagnosed with early stage(stageⅠ-Ⅱ)nasopharyngeal carcinoma,and 43 patients with middle to late stage(stageⅢ-Ⅳ)nasopharyngeal carcinoma.The influencing factors related to clinical staging of nasopharyngeal carcinoma were analyzed,and the basic information,clinical symptoms,lymph node metastasis,tumor growth patterns of patients with different clinical stages were compared differences in pathological types and other aspects.Results There was no significant difference in the ratio of male to female,age,growth pattern and pathological type between patients with stageⅠ-Ⅱand stageⅢ-Ⅳnasopharyngeal carcinoma(P>0.05).There were significant differences in the first symptoms,accompanying symptoms and lymph node metastasis between patients with stageⅠ-Ⅱand stageⅢ-Ⅳnasopharyngeal carcinoma(P<0.05).There were significant differences in serological indicators(VCA-IgA,CEA,CYFRA21-1)between patients with stageⅠ-Ⅱand stageⅢ-Ⅳnasopharyngeal carcinoma(P<0.05).Through logistic multiple factor analysis,the first symptom(OR=3.704),accompanying symptoms(OR=4.229),lymph node metastasis(OR=7.702),VCA-IgA(OR=7.318),CEA(OR=7.512),and CYFRA21-1(OR=7.318)are related factors that affect the clinical staging of nasopharyngeal carcinoma.Conclusion The initial symptoms,accompanying symptoms,lymph node metastasis,VCA-IgA,CEA and CYFRA21-1 are the related factors affecting the clinical stage of nasopharyngeal carcinoma.According to the clinical characteristics of patients with different clinical stages in terms of clinical symptoms and lymph node metastasis,the relevant factors of clinical stage can be clarified,and a targeted diagnosis and treatment plan can be formulated.
