摘要
目的探讨胰岛素联合雌激素软膏治疗糖尿病伴阴道炎的临床效果。方法选取82例厦门大学附属第一医院杏林分院2020年3月—2022年3月收治的糖尿病伴阴道炎患者,按随机数表法分为两组,各41例。两组均予以雌激素软膏,对照组加用二甲双胍治疗,观察组加用胰岛素治疗,持续治疗4周。比较两组治疗效果。结果观察组总有效率较对照组高,阴道瘙痒(4.69±1.08)d、阴道灼痛(3.98±0.75)d、分泌物增多(6.12±1.13)d及充血水肿(7.89±1.25)d缓解时间短于对照组,差异有统计学意义(P<0.05);观察组治疗后空腹血糖(FPG)(5.58±0.85)mmol/L、糖化血红蛋白(HbA1c)(6.29±1.05)%、餐后2 h血糖(2 hPG)水平(8.09±1.05)mmol/L低于对照组,差异有统计学意义(P<0.05);观察组治疗后空腹胰岛素(FINS)(4.93±0.82)mIU/L、胰岛素抵抗指数(HOMA-IR)水平(2.48±0.35)较对照组低,胰岛β细胞功能指数(HOMA-β)水平(42.18±4.38)较对照组高,差异有统计学意义(P<0.05);两组不良反应比较,差异无统计学意义(P<0.05)。结论胰岛素联合雌激素软膏可提高糖尿病伴阴道炎治疗效果,加快血糖水平复常,减轻胰岛功能障碍,促进阴道炎症状消退,安全可靠。
Objective To investigate the clinical effect of insulin combined with estrogen ointment in the treatment of diabetes mellitus with vaginitis.Methods Eighty-two diabetes mellitus patients with vaginitis admitted to the Xinglin Branch of the First Affiliated Hospital of Xiamen University from March 2020 to March 2022 were selected and divided into two groups of forty-one cases each according to the random number table method.Both groups were given estrogen ointment,and the control group was treated with metformin and the observation group was treated with insulin for 4 weeks.The treatment effect of the two groups was compared.Results The overall efficiency of the observation group was higher than that of the control group,and the time to relief of vaginal itching(4.69±1.08)d,vaginal burning pain(3.98±0.75)d,increased discharge(6.12±1.13)d and congestion and edema(7.89±1.25)dwas shorter than those of the control group,the difference was statistically significant(P<0.05).Fasting blood glucose(FPG)(5.58±0.85)mmol/L,glycosylated hemoglobin(HbA1c)(6.29±1.05)%and 2h postprandial glucose(2 hPG)levels(8.09±1.05)mmol/L in the observation group were lower than those in the control group after treatment,with statistical significant differences(P<0.05).Fasting insulin(FINS)(4.93±0.82)mIU/L and insulin resistance index(HOMA-IR)levels(2.48±0.35)were lower and pancreaticβ-cell function index(HOMA-β)levels(42.18±4.38)in the observation group were higher than those in the control group after treatment,with a statistical statistically difference(P<0.05).There was no statistically significant difference in adverse reactions between the two groups(P>0.05).Conclusion Insulin combined with estrogen ointment can improve the therapeutic effect of diabetes mellitus with vaginitis,accelerate the normalization of blood glucose level,reduce islet dysfunction,and promote the regression of vaginitis symptoms,which is safe and reliable.
