摘要
目的通过随机分组方法研究临床思辨教学在口腔全科住院医师规范化培训中的教学效果,以及临床思辨教学对于提升口腔全科住院医师整体临床诊疗能力是否具有积极促进作用。方法将2019年3月—2020年3月以及2020年3月—2021年3月的在空军军医大学急诊与临床综合科参加住院医师规范化培训的75名住院医师分别设为试验组与对照组,分别按照临床思辨教学模式与传统教学模式进行带教,并对综合临床能力进行考核。结果教学考核结果显示,两组客观题考核结果差异无统计学意义(P>0.05),而临床思辨考核结果试验组成绩(84.54±3.66)分,明显优于对照组(78.78±4.35)分,差异有统计学意义(P<0.05),试验组学员在整体诊疗能力方面,牙体治疗和修复治疗成绩分别为(42.59±2.42)分和(41.77±2.31)分,明显优于对照组(38.11±1.97)分和(37.63±2.03)分,差异有统计学意义(P<0.05)。结论临床思辨模式相较于传统带教模式,有助于在规范化培训中提升住培医师的临床思辨能力与综合临床实践能力,但对于提升基础理论知识水平并无显著效果。
Objective To investigate whether speculative teaching contributes to the promotion of clinical speculative ability and generally clinical ability in standard training of stomatology residents who divided into two groups randomly.Methods A total of 75 residents who participated in the standardized training of resident doctors in the emergency and clinical general department of Air Force Medical University from March 2019 to March 2020 and from March 2020 to March 2021 were set as the experimental group and the control group,respectively.They were taught according to the clinical critical teaching mode and the traditional teaching mode,and the comprehensive clinical ability was assessed.Results Teaching assessment results showed that there was no statistically significant difference between the two groups in objective question assessment results(P>0.05),while clinical critical assessment results of the experimental group(84.54±3.66)points,significantly better than the control group(78.78±4.35)points,the difference was statistically significant(P<0.05).The results of dental treatment and prosthesis treatment were(42.59±2.42)points and(41.77±2.31)points,respectively,which were significantly better than the control group(38.11±1.97)points and(37.63±2.03)points,and the difference was statistically significant(P<0.05).Conclusions Speculative teaching play an important role in promoting the clinical speculative ability and treating ability whereas it has no effect in basic clinical theories.
