摘要
目的 分析帕金森病(PD)和血管性帕金森综合征(VP)的颅脑磁共振成像(MRI)影像学特征。方法 将2019年5月至2021年5月本院收治的30例PD患者作为PD组,30例VP患者作为VP组,30名健康人群作为对照组进行颅脑MRI检查。比较三组的黑质致密带宽度(WPCSN)、中脑直径、WPCSN/中脑直径;比较PD组和VP组不同病情严重程度的WPCSN、中脑直径、WPCSN/中脑直径;比较PD组和VP组的颅脑病变检测结果;分析PD组和VP组的颅脑MRI影像学表现。结果 PD组的WPCSN、WPCSN/中脑直径低于VP组和对照组(P<0.05);VP组和对照组的WPCSN、WPCSN/中脑直径比较,差异无统计学意义(P>0.05);三组的中脑直径比较,差异无统计学意义(P>0.05)。PD组Ⅰ~Ⅱ、Ⅲ、Ⅳ级的WPCSN低于VP组,差异具有统计学意义(P<0.05)。PD组的基底节病变、皮层下白质病变、下丘脑病变、脑干损伤检出率低于VP组,差异具有统计学意义(P<0.05)。PD组颅脑MRI影像学检查显示无特殊变化;VP组颅脑MRI影像学检查显示颅脑缺血性变化异常信号,T_(1)加权成像(T_(1)WI)为低信号或等信号,T_(2)加权成像(T_(2)WI)为高信号,主要区域为基底节区、侧脑室旁脑白质等。结论 颅脑MRI对PD和VP进行鉴别诊断有一定的临床意义,VP的脑损伤较PD更严重,影像学病理特征更为明显。
Objective To analyze the brain magnetic resonance imaging(MRI)imaging features of Parkinson's disease(PD)and vascular Parkinson's syndrome(VP).Methods Thirty PD patients admitted in our hospital from May 2019 to May 2021 were selected as PD group,30 VP patients as VP group,and 30 healthy people as control group.The width of pars compacta of substantia nigra(WPCSN),midbrain diameter,WPCSN/midbrain diameter were compared among the three groups;the WPCSN,midbrain diameter,WPCSN/midbrain diameter of different severity were compared between the PD group and the VP group;the detection results of craniocerebral lesions were compared between the PD group and the VP group;the brain MRI imaging findings of the PD group and the VP group were analyzed.Results The WPCSN and WPCSN/midbrain diameters in the PD group were lower than those in the VP group and the control group(P<0.05);there were no significant difference in WPCSN and WPCSN/midbrain diameter between the VP group and the control group(P>0.05);there was no significant difference in midbrain diameter among the three groups(P>0.05).The WPCSN of patients with gradeⅠ-Ⅱ,ⅢandⅣin the PD group were lower than those in the VP group,and the differences were statistically significant(P<0.05).The detection rates of basal ganglia lesions,subcortical white matter lesions,hypothalamic lesions and brainstem injury in the PD group were lower than those in the VP group,and the differences were statistically significant(P<0.05).There was no special change in brain MRI imaging examination in the PD group.In the VP group,brain MRI imaging examination showed abnormal signal of cerebral ischemic changes,T_(1) weighted imaging(T_(1)WI)showed low signal or equal signal,and T_(2) weighted imaging(T_(2)WI)showed high signal,and the main areas were basal ganglia,paraventricular white matter,etc.Conclusion Brain MRI has certain clinical significance in differential diagnosis of PD and VP;the brain injury of VP is more serious than that of PD,and its imaging pathological features are mor
