磁共振人工智能脑分割软件定量评估阿尔茨海默病患者不同脑区体积的变化规律 被引量:2

摘要 目的研究磁共振人工智能脑分割软件精准定位并测量阿尔茨海默病(AD)患者不同脑区的体积变化规律,为疾病发生机制提供参考依据。方法纳入66例AD患者为研究对象,应用上海联影1.43T uMR586磁共振扫描头颅,并采用人工智能脑分割软件测量各脑区体积的变化规律。结果全脑、白质、外侧颞叶区、枕叶区、扣带回区、岛叶区和小脑体积及百分比均在正常参考范围内,而灰质、内侧颞叶区、额叶区、顶叶区和皮层下灰质区体积及百分比均低于正常参考范围,脑室区体积及百分比均高于正常参考范围(t分别=5.97、5.00、10.32、7.64、5.97;5.66、4.96、4.52、4.75、4.32、-20.32、-7.62,P均<0.05)。常规MRI未见明显脑萎缩表现。进一步发现,内侧颞叶区的海马体、海马旁回和内嗅皮质体积及百分比明显低于正常参考范围(t分别=6.63、5.00、5.12;5.65、5.23、5.29,P均<0.05),而杏仁核、梭状回和颞极体积及百分比在正常参考范围。额叶区、顶叶区和皮层下灰质区在亚区各部分体积及百分均在正常参考范围。脑室区的侧脑室和第三脑室体积及百分比明显高于正常参考范围(t分别=6.32、4.85;5.75、5.06,P均<0.05),第四脑室和外周脑脊液的体积及百分比在正常参考范围内。内侧颞叶萎缩视觉(MTA)评分与MMSE评分的相关性较好(r=0.46,P<0.05)。结论磁共振人工智能脑分割软件能够精准定位并测量不同脑区的体积变化规律,AD患者表现为以大脑灰质、海马和内嗅皮质萎缩以及脑室扩张为重要特征,为疾病发生机制提供了重要理论依据。 Objective To study the volume change of different brain regions in patients with Alzheimer's disease(AD)accurately located and measured by magnetic resonance artificial intelligence brain segmentation software,so as to provide a reference for the pathogenesis of disease.Methods A total of 66 patients with AD were enrolled.After admis⁃sion,Shanghai Lianying 1.43T uMR586 was used to scan head and artificial intelligence brain segmentation software was used to measure the volume change rule of each brain region.Results The volume and percentage of the whole brain,white matter,lateral temporal lobe,occipital lobe,cingulate gyrus,insular lobe and cerebellum were all within the normal reference range,while the volume and percentage of gray matter,medial temporal lobe,frontal lobe,pariet al lobe and subcortical gray matter were lower than the normal reference range,while the volume and percentage of ventricular area and their percentage were higher than the normal reference range(t=5.97,5.00,10.32,7.64,5.97,5.66,4.96,4.52,4.75,4.32,-20.32,-7.62,P<0.05).No obvious brain atrophy was found in conventional MRI.The volume and percentage of hippocampus,parahippocampal gyrus and entorhinal cortex in medial temporal lobe area and their percentage were signif⁃icantly lower than the normal reference range(t=6.63,5.00,5.12,5.65,5.23,5.29,P<0.05),while the volume and per⁃centage of amygdala,fusiform gyrus and temporal pole were in the normal reference range.The vol⁃ume and percentage of frontal lobe,parietal lobe and subcortical gray matter area and their percent⁃age were in the normal reference range.What’s more,the volume and percentage of lateral ventricle and the third ventricle in the ventricular area were significantly high⁃er than the normal reference range(t=6.32,4.85,5.75,5.06,P<0.05),while the volume and percentage of the fourth ventricle and peripheral cerebrospinal fluid were in the normal reference range.Pearson correlation analysis showed that the medial temporal lobe atrophy visual(MTA)score was positiv
