不同频率反馈式呼吸电刺激训练促进中重度COPD稳定期患者的康复效果对比

摘要 目的对比分析不同反馈式呼吸电刺激对促进中重度慢性阻塞性肺疾病(COPD)稳定期患者康复的效果。方法选取2019年7月至2022年7月平煤神马医疗集团总医院收治的94例中重度COPD稳定期患者,按随机数字表法分为观察组与对照组,各47例。两组均接受常规治疗,对照组接受10 Hz频率反馈式呼吸电刺激,观察组接受30 Hz反馈式呼吸电刺激。对比两组肺功能[第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)]、6 min步行距离(6MWD)、膈肌活动度[平静呼吸时膈肌活动度(QDM)、用力呼吸膈肌活动度(FDM)]、呼吸肌功能[最大吸气压(MIP)、最大呼气压(MEP)]、呼吸困难量表评分(MRC)、圣乔治呼吸问卷评分(SGRQ)。结果干预后,观察组FEV_(1)、FVC较对照组高(P<0.05)。干预后,观察组6MWD较对照组长(P<0.05)。干预后,观察组QDM、FDM活动度较对照组高(P<0.05)。干预后,观察组MEP、MIP较对照组高(P<0.05)。干预后,观察组MRC、SGRQ评分较对照组低(P<0.05)。结论高频率反馈式呼吸电刺激应用于中重度COPD稳定期患者中,可加快患者康复,改善肺功能、膈肌活动度和呼吸肌功能,可提高运动耐力,减轻对生活质量的影响。 Objective To compare and analyze the effect of different feedback electrical respiratory stimulation on the recovery of stable pulmonary disease in patients with moderate and severe chronic obstructive pulmonary disease(COPD).Methods A total of 94 patients with moderate and severe COPD in General Hospital of Pingmei Shenma Group from July 2019 to July 2022 were selected and divided into observation group and control group according to random number table method,with 47 cases in each group.Both groups received conventional treatment,the control group received 10 Hz frequency feedback electrical respiratory stimulation,the observation group received 30 Hz feedback electrical respiratory stimulation.The lung function[forced expiratory volume in the first second(FEV 1),forced vital capacity(FVC)],6-minute walking distance(6MWD),diaphragm activity[quiet diaphragm muscle mobility(QDM),forced diaphragm muscle mobility(FDM)],respiratory muscle function[maximal inspiratory pressure(MIP),maximal expiratory pressure(MEP)],Medical Research Council dyspnea scale(MRC)and St George’s respiratory questionnaire(SGRQ)were compared between the two groups.Results After intervention,FEV 1 and FVC in observation group were higher than those in control group(P<0.05).After intervention,6MWD in observation group was longer than that in control group(P<0.05).After intervention,the activity of QDM and FDM in observation group was higher than those in control group(P<0.05).After intervention,MEP and MIP in observation group were higher than those in control group(P<0.05).After intervention,MRC and SGRQ scores in observation group were lower than those in control group(P<0.05).Conclusion The application of respiratory electrical stimulation training with high frequency feedback in moderate and severe COPD patients can accelerate the recovery of patients,improve lung function,diaphragm activity and respiratory muscle function,improve exercise endurance,and reduce the impact on quality of life.
