某石油化工企业工人代谢综合征与限制性肺通气功能障碍的关联性分析

Association between components of metabolic syndrome and restrictive pulmonary ventilation dysfunction in an enterprise population in Lanzhou

摘要 目的分析某石油化工(简称石化)企业工人代谢综合征各组分与限制性肺通气功能障碍的相关性。方法对316名石化企业职工进行职业健康体检和问卷调查,建立logistic回归模型分析代谢综合征组分对限制性肺通气功能障碍的影响。结果单因素分析显示,男性占比、吸烟率、岗位接触化学物质比例、高密度脂蛋白值在肺通气障碍组均高于肺通气正常组,低密度脂蛋白值在肺通气正常组高于肺通气障碍组,但差异均无统计学意义(χ^(2)=0.09、1.23、0.69,t=0.53、0.26,均P>0.05);高血压占比、超重/肥胖占比、血脂异常率、甘油三酯值、空腹血糖值在肺通气障碍组均高于肺通气正常组,差异有统计学意义(χ^(2)=6.92、16.57、13.58,Z=2.21、4.68,均P<0.05)。多因素logistic回归模型显示,超重及肥胖、血脂紊乱、空腹血糖升高都会增加限制性肺通气障碍发生的风险,差异有统计学意义[OR(95%CI):1.861(1.039,3.333)、3.536(1.703,7.342)、1.399(1.135,1.724),均P<0.05]。不同年龄亚组代谢组分累积分布差异有统计学意义。(Z=-2.68、-3.99,均P<0.05)。结论超重及肥胖、血脂紊乱、高血糖是石化工人限制性肺通气功能障碍发生的危险因素。 Objective To analyze the correlation between the components of metabolic syndrome and restricted pulmonary ventilation dysfunction of workers in a petrochemical enterprise.Methods Occupational health examination and questionnaire survey were conducted among 316 workers in petrochemical enterprises.Logistic regression model was established to analyze the effects of metabolic syndrome components on restricted pulmonary ventilation dysfunction.Results Univariate analysis showed that the proportion of males,smoking rate,the proportion of job exposure to chemicals and the high density lipoprotein value in the lung ventilation disorder group were all higher than those in the normal lung ventilation group,and the low density lipoprotein value in the normal lung ventilation group was higher than that in the abnormal lung ventilation group,but the differences were not statistically significant(χ^(2)=0.09,1.23,0.69,t=0.53,0.26,all P>0.05).The proportion of hypertension,the proportion of overweight/obesity,the rate of dyslipidemia,the value of triglyceride and the value of fasting blood glucose in the pulmonary ventilation disorder group were higher than those in the normal pulmonary ventilation group,and the difference was statistically significant(χ^(2)=6.92,16.57,13.58,Z=2.21,4.68,P<0.05).Multivariate logistic regression model showed that overweight and obesity,dyslipidemia and elevated fasting blood glucose all increased the risk of restrictive pulmonary ventilation disorder,and the difference was statistically significant[OR(95%CI):1.861(1.039,3.333),3.536(1.703,7.342),1.399(1.135,1.724),P<0.05].There were differences in metabolic components aggregation between the two groups in different years(Z=-2.68,-3.99,P<0.05).Conclusion Overweight and obesity,dyslipidemia and hyperglycemia are risk factors of restrictive pulmonary ventilation dysfunction in petrochemical workers.
