摘要
目的:评估碱性磷酸酶(alkaline phosphatase,ALP)与甲胎蛋白(alpha fetoprotein,AFP)异质体(AFP-L3)比率(AFP-L3%)联合检测诊断原发性肝癌(primary hepatic carcinoma,PHC)的价值。方法:选取2021年1月—2022年1月就诊于徐州市贾汪区人民医院诊断为PHC的60例患者作为PHC组,诊断为肝硬化(liver cirrhosis,LC)的60例患者作为LC组。收集两组一般资料,检测两组实验室指标。比较两组一般资料及实验室指标,并对PHC进行多因素分析。分析ALP及AFP-L3%诊断PHC的价值。结果:PHC组年龄、血清白蛋白、ALP、AFP-L3%水平均明显高于LC组,丙氨酸氨基转移酶(alanine aminotransferase,ALT)、天门冬酸氨基转移酶(aspartate aminotransferase,AST)、总胆红素水平均明显低于LC组,差异有统计学意义(P<0.05)。logistic回归分析结果显示,年龄、ALP、AFP-L3%是PHC的危险因素(P<0.05)。ALP预测PHC的约登指数为0.388,敏感度为65.6%,特异度为73.2%;AFP-L3%预测PHC的约登指数为0.652,敏感度为79.6%,特异度为85.6%。ALP联合AFP-L3%预测PHC的约登指数为0.560,敏感度为84.9%,特异度为71.1%,ALP和AFP-L3%联合检测的AUC值最高。结论:ALP及AFP-L3%联合检测能有效提高PHC诊断价值。
Objective:To evaluate the value of combined detection of alkaline phosphatase(ALP)and alpha fetoprotein(AFP)heteroplasmon(AFP-L3)ratio(AFP-L3%)in the diagnosis of primary hepatic carcinoma(PHC).Method:A total of 60 patients diagnosed with PHC who were treated in Jiawang District People's Hospital of Xuzhou City from January 2021 to January 2022 were selected as PHC group and 60 patients diagnosed with liver cirrhosis(LC)as LC group.General data of two groups were collected and laboratory indicators of two groups were tested.General data and laboratory indicators were compared between the two groups,and multivariate analysis was performed for PHC.The diagnostic value of ALP and AFP-L3%in PHC was analyzed.Result:The age,serum albumin,ALP,and AFP-L3%levels of the PHC group were significantly higher than those of the LC group,and the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin were significantly lower than those in LC group,and the differences were statistically significant(P<0.05).logistic regression analysis showed that age,ALP and AFP-L3%were risk factors for PHC(P<0.05).ALP predicted PHC with a Yoden index of 0.388,a sensitivity of 65.6%and a specificity of 73.2%.AFP-L3%predicted PHC with a Yoden index of 0.652,a sensitivity of 79.6%,and a specificity of 85.6%.ALP combined with AFP-L3%predicted PHC with a Yoden index of 0.560,a sensitivity of 84.9%,and a specificity of 71.1%.The AUC value of ALP combined with AFP-L3%detection was the highest.Conclusion:Combined detection of ALP and AFP-L3%can effectively improve the diagnostic value of PHC.
