针刺对脑卒中偏瘫恢复期患者正中神经电生理的变化 被引量:1

摘要 目的:观察针刺对脑卒中偏瘫恢复期患者正中神经的电生理变化。方法:对照组采用常规康复训练,研究组在对照组的基础上加用针刺治疗,每14d为一个疗程,4个疗程后评定疗效。结果:所有疗程结束后,两组患者治疗后的NIHSS评分均有降低,BI、FMA、FMB评分均有升高(P<0.05),且研究组NIHSS评分<对照组(P<0.05),BI、FMA、FMB评分大于对照组(P<0.05)。研究组正中神经运动传导和感觉传导速度均较前增加,其中脘及肘-脘部正中神经运动传导电位波幅均较前增大,潜伏期减少;感觉传导电位波幅也较前增大,潜伏期减少(P<0.05)。且研究组在治疗后各项指标均显著差异于对照组(P<0.05)。结论:结果表明单纯康复治疗,或联合针刺均能改善偏瘫患者临床症状,但后者疗效更佳;针刺联合康复训练较单一的康复训练改变偏瘫患者的正中神经电生理变化更加显著。 Objective:To observe the electrophysiological changes of the median nerve in stroke patients with hemiplegia.Methods:The control group received routine rehabilitation training,and the research group received acupuncture on the basis of the control group.Every 14 days was a course of treatment,and the curative effect was evaluated after 4 courses of treatment.Results:After all courses of treatment,the NIHSS scores of the two groups decreased,and the BI,FMA,and FMB scores all increased(P<0.05),and the NIHSS scores of the research group were lower than those of the control group(P<0.05),and the BI,FMA,and FMB scores were higher than those in the control group(P<0.05).The motor conduction and sensory conduction velocities of the median nerve in the study group were both increased compared with before,and the motor conduction potential amplitudes of the epigastric and elbow-epilateral median nerves were all increased and the latency was decreased;the sensory conduction potential amplitudes were also increased and the latency was decreased(P<0.05).And the indexes of the study group after treatment were significantly different from those of the control group(P<0.05).Conclusion:The results show that rehabilitation alone or combined with acupuncture can improve the clinical symptoms of patients with hemiplegia,but the latter is more effective.Acupuncture combined with rehabilitation training can change the electrophysiological changes of the median nerve in hemiplegic patients more significantly than single rehabilitation training.
