摘要
目的:综合分析古代先贤对升降散的论述以及现代关于升降散治疗气郁化火型偏头痛的临床研究,探究升降散治疗气郁化火型偏头痛的作用机制,以期为临床治疗提供新思路。方法:通过研究血管学说、三叉神经学说、炎症介质学说等三大主流学说中偏头痛的病理机制及辨证,探析气郁化火型偏头痛的病因病机,分析升降散的现代药理机制及功效,从而归纳升降散治疗偏头痛的治法原则。结果:气郁化火型偏头痛的病机为“气机郁滞,久郁化火,上扰清窍,而致清阳不升,浊阴不降”,升降散可“宣郁泄热,降阴中之浊阴,升阳中之清阳”,从而改善偏头痛。结论:升降散可“通利上下,调和内外”,对气郁化火型偏头痛有较好的临床疗效。
Objective:To comprehensively analyze the ancient sages' expositions on Shengjiang powder and modern clinical research on the treatment of Qi stagnation and Huahuo type migraine with Shengjiang powder,and to explore the mechanism of Shengjiang powder in the treatment of Qi stagnation and Huahuo migraine,in order to provide new clinical treatment.train of thought.Methods:By studying the pathological mechanism of migraine in the three mainstream theories of blood vessel theory,trigeminal nerve theory and inflammatory mediator theory,and dialectically analyzing the etiology and pathogenesis of migraine of Qi stagnation and turning into fire type,and analyzing the modern pharmacological mechanism and efficacy of Jiangjiang San,Thus summed up the principle of treatment of migraine with Shengjiang Powder.Results:The pathogenesis of migraine of qi stagnation and fire type is “stagnation of Qi,long-term stagnation of fire,disturbance of clear orifices,resulting in the failure of clear yang to rise and turbid yin not to fall”.,turbid yin in lowering yin,clear yang in raising Yang”,thereby improving migraine.Conclusion:Shengjiang powder can “Tongli up and down,harmonize internal and external”,and it has a good clinical effect on migraine of Qi stagnation and fire type.
