摘要
目的了解南宁市健康人群麻疹、风疹、流行性腮腺炎(流腮)的免疫水平及其影响因素,为制定防控措施提供科学依据。方法2017-2019年对南宁市7个县区不同年龄段的健康人群进行调查,收集其年龄、民族、免疫史等相关资料,采集2~3 mL静脉血进行抗体浓度检测,计算抗体的阳性率、保护率及浓度中位数。采用logistic回归模型分析抗体阳性率和保护率的影响因素。结果1322名调查对象中,麻疹、风疹、流腮抗体浓度中位数分别为861.86 mIU/mL、59.07 IU/mL、371.81 IU/mL,阳性率分别为91.23%、84.87%、77.9%,麻疹抗体保护率为54.54%。麻疹、风疹、流腮的抗体浓度中位数在不同县区、性别、年龄、民族、免疫史和免疫时间间隔之间,差异有统计学意义,均P<0.05;麻疹抗体阳性率和保护率在不同县区、年龄、免疫史、免疫时间间隔之间,差异有统计学意义,均P<0.05;风疹抗体阳性率在不同县区、年龄、民族、免疫史、免疫时间间隔之间,差异有统计学意义,均P<0.05;流腮抗体阳性率在不同县区、性别、年龄、免疫史之间,差异有统计学意义,均P<0.05。多因素分析结果显示,年龄(OR=1.02,95%CI为1.01~1.04,P<0.001)、免疫史(OR=1.09,95%CI为1.07~1.11,P<0.001)是麻疹抗体阳性率的影响因素,年龄(OR=0.95,95%CI为0.92~0.97,P<0.001)、免疫史(OR=1.14,95%CI为1.10~1.18,P<0.001)、免疫时间间隔(OR=1.01,95%CI为1.01~1.02,P<0.001)是麻疹抗体保护率的影响因素,县区(OR=0.99,95%CI为0.98~1.00,P=0.031)是风疹抗体阳性率的影响因素,县区(OR=0.98,95%CI为0.97~1.00,P=0.007)、年龄(OR=1.03,95%CI为1.02~1.04,P<0.001)、免疫史(OR=1.15,95%CI为1.12~1.18,P<0.001)是流腮抗体阳性率的影响因素。结论南宁市麻疹、风疹抗体阳性率接近消除的要求,流腮抗体阳性率较低。麻疹和风疹及流腮的抗体水平受地区、年龄、免疫史、免疫时间间隔等不同程度影响,麻疹、风疹呈现衰退情况。建
Objective This study aimed to investigate measles,rubella and mumps serum antibody level and its associated influencing factors for the healthy population in Nanning city,and provide evidence to prevention and control measures in the future.Methods We conducted a cross-sectional survey by convenience sample on healthy people among different ages at 7 regions of Nanning city in 2017-2019.Information,including age,nationality,immune history and other relevant data were investigated,and 2-3 ml venous blood was collected for antibody concentration detection.The positive rate,protective rate,and median concentration of antibodies were calculated.Multivariate logistic modal was used to analyze the influencing factors of antibody positive and protective rates.Results Of the 1322 study subjects,the median concentrations of antibodies against measles,rubella,and mumps were 861.86 mIU/ml,59.07 IU/ml,and 371.81 IU/ml,and the antibodies positive rate were 91.23%,84.87%and 77.9%,respectively.The protection rate of measles antibodies was 54.54%.The median antibody concentrations of measles,rubella,and mumps were statistically significant among different counties,regions,gender,age,ethnicity,immunization history,and immunization time intervals,with all P<o.05;The positive rate and protective rate of measles antibody showed statistical significance among different counties,ages,immune histories,and immune interval,with all P<o.05;The positive rate of rubella antibodies showed statistical significance among different counties,regions,ages,ethnic groups,immune histories,and immune interval,with all P<o.05;The positive rate of mumps antibody showed statistical significance among different counties,regions,genders,ages,and immune histories,with all P<0.05.The results of multivariate analysis showed that age(OR=1.02,95%CI:1.01-1.04,P<0.001)and immune history(OR=1.09,95%CI:1.07-1.11,P<0.001)were the influencing factors for measles antibody positivity,while age(OR=0.95,95%CI:0.92-0.97,P<0.001),immune history(OR=1.14,95%CI:1.10-1.18,P<0
