MYH9在胶质瘤中的表达及临床预后的生物信息学研究

摘要 目的探讨在胶质瘤中非肌性肌球蛋白重链9(MYH9)的表达水平及对肿瘤进展与预后的临床意义。方法基于中国脑胶质瘤基因组图谱计划CGGA数据库中具有完整临床病理及随访资料的胶质瘤组织病例656例,收集并选取病例的基因测序结果及对应的临床病理资料,分析MYH9在不同级别胶质瘤组织中的表达特点,以及与临床病理参数的关系。应用Kaplan-Meier生存分析MYH9表达状态与胶质瘤病例生存时间的关系。采用单因素及多因素分析确定MYH9表达水平是否是胶质瘤患者预后的独立危险因素。结果MYH9在低级别胶质瘤中的表达水平明显低于高级别胶质瘤(P<0.0001),MYH9表达水平与Ki-67的表达水平呈明显正相关(r=0.2978,P<0.0001)。MYH9表达情况与胶质瘤患者病理类型、临床WHO分级、Ki-67表达状态、IDH突变及1p/19q联合缺失情况密切相关(P<0.001)。Kaplan-Meier生存分析结果显示,MYH9高表达状态的病例生存时间明显低于低表达状态的病人(P<0.0001)。Cox回归单因素及多因素分析结果显示MYH9表达状态与患者生存预后明显相关,其表达情况是胶质瘤患者生存预后的独立危险因素(P<0.001,HR=1.671)。结论MYH9在高级别胶质瘤中呈高表达状态,且其表达状态是胶质瘤恶性进展和预后不良的潜在因素。 Objective To investigate the expression level and clinical significance for the malignant progression and prognosis of myosin heavy chain 9(MYH9)in human glioma.Methods Based on 656 cases of glioma tissue with complete clinicopathological and follow-up data from The Chinese Glioma Genome Atlas database,gene sequencing results and corresponding clinicopathological data were collected and extracted to analyze the expression characteristics of MYH9 in different grade tissues and its relationship with clinicopathological parameters.Kaplan-meier survival analysis was used to analyze the relationship between MYH9 expression level and survival time of patients with glioma.Univariate and multivariate analyses were used to determine whether MYH9 expression level was an independent risk factor for prognosis in patients with glioma.Results The expression level of MYH9 in low grade glioma was significantly lower than that in high grade glioma tissue(P<0.0001),and there was a significant positive correlation between MYH9 expression level and Ki-67 expression level(r=0.2978,P<0.0001).MYH9 expression level was closely correlated with the pathological type,WHO grade,Ki-67 expression level,IDH mutation and 1p/19q codeletion status(P<0.001).Kaplan-meier survival analysis showed that the survival time of patients with low MYH9 expression was significantly longer than that of patients with high expression(P<0.0001).Cox regression univariate and multivariate analysis showed that MYH9 expression was significantly correlated with survival,and MYH9 expression status was an independent risk factor for survival of patients with glioma(P<0.001,HR=1.671).Conclusion Our study indicated that MYH9 was significantly highly expressed in high grade glioma,and its expression status was a potential factor of disease progression and poor prognosis in glioma.
