摘要
目的探究自由体位助产联合分娩球对初产妇分娩结局、生殖道损伤情况及满意度的影响。方法选取2020年1月—2021年3月于广东省江门市妇幼保健院产科分娩的109例初产妇为研究对象,采用随机数字表法将其分成对照组(55例)与观察组(54例)。对照组实施常规分娩护理,观察组实施自由体位助产联合分娩球干预。比较两组初产妇的分娩结局(分娩方式、总产程、产后2 h出血量)、产痛情况及生殖道损伤情况、分娩控制量表评分、护理质量评分、新生儿Apgar评分及满意度。结果观察组初产妇的分娩结局、产痛情况、生殖道损伤情况均显著优于对照组初产妇(P<0.05);观察组初产妇的分娩控制量表评分、护理质量评分、新生儿Apgar评分均高于对照组(P<0.05);观察组初产妇的满意度高于对照组初产妇(P<0.05)。结论实施自由体位助产与分娩球可较好地改善初产妇的分娩结局,缓解分娩疼痛,提升护理质量,减轻生殖道损伤,改善新生儿结局,提升产妇的满意率。
Objective To explore the effect of free position midwifery combined with birthing ball on delivery outcome,genital tract injury and satisfaction of primipara.Methods A total of 109 primiparas who gave birth in the obstetrics department of Jiangmen Maternity and Child Health Hospital of Guangdong Province from January 2020 to March 2021 were selected as the research objects,and were divided into control group(55 cases)and observation group(54 cases)by random number table method.The control group received routine delivery nursing,and the observation group received free position midwifery combined with birthing ball intervention.The outcomes of delivery(mode of delivery,total labor,2 h postpartum bleed loss),labor pain,genital tract injury,Labour Agentry scale score,nursing quality score,neonatal Apgar score and satisfaction were compared between the two groups.Results The outcomes of delivery,labor pain and genital tract injury in the observation group were significantly better than those in the control group(P<0.05).The Labour Agentry scale score,nursing quality score and Apgar score of neonates in the observation group were higher than those in the control group(P<0.05).The satisfaction of primiparas in the observation group was higher than that in the control group(P<0.05).Conclusion The implementation of free position midwifery and birthing ball can better improve the delivery outcome of primipara,relieve labor pain,improve nursing quality,reduce reproductive tract injury,improve neonatal outcome,and increase maternal satisfaction.
