摘要
目的分析使用UpToDate临床顾问(简称UpToDate)获益得分的影响因素,为住院医师的循证医学培训提供参考。方法采用问卷进行抽样调查,并用随机森林算法分析UpToDate获益得分的影响因素以及变量的偏依赖性。结果通过均方误差的平均递减对变量重要性排序,结果显示影响使用UpToDate获益得分排序前3的变量为修订版Jefferson医生终身学习量表得分、UpToDate对临床决策的影响及1周内使用UpToDate的频次。随着修订版Jefferson医生终身学习量表得分的增加,使用UpToDate获益得分呈波动上升趋势,随着使用UpToDate改变临床决策得分的增加,使用UpToDate获益得分也呈现上升趋势。1周内使用UpToDate4~5次,获益得分达到最高。结论应加强住院医师终身学习能力培养,鼓励住院医师使用UpToDate梳理医学知识,增强住院医师将新信息整合到临床实践中的信心,适宜提高UpToDate使用频次,令使用UpToDate获益最大化。
Objective To analyze the influencing factors of the benefit score of using UpToDate and to provide a reference for the evidence-based medicine training of residents.Methods A questionnaire was used to conduct a sampling survey,and the random forest algorithm was used to analyze the influencing factors of the benefit score of using UpToDate and the partial dependence on the influencing factors.Results The importance of variables was ranked by the mean decrease of mean square error.The results showed that the top three variables affecting the benefit score of using UpToDate were the Revised Jefferson Scale of Physician Lifelong Learning score,the impact score of UpToDate on clinical decision-making and the frequency of using UpToDate within a week.With the increase of the Revised Jefferson Scale of Physician Lifelong Learning score,the benefit score of using UpToDate showed a fluctuating upward trend.As the impact of UpToDate on clinical decision-making score increased,the UpToDate Benefit Score also trended upward.Using UpToDate 4-5 times in a week achieved the highest benefit score of using UpToDate.Conclusion It is necessary to strengthen the training of residents'lifelong learning ability,to encourage residents to use UpToDate to organize medical knowledge,to enhance residents'confidence in integrating new information into their clinical practice,and to appropriately increase the frequency of using UpToDate to maximize the benefits.
