摘要
目的运用神经电生理检查对维持性血液透析(MHD)患者的周围神经功能状态进行客观评价,并评价运动康复训练对该类患者周围神经功能及疲劳状态的影响,制定可操作性、简便性的运动策略。方法选取2022年1月至6月潍坊医学院附属医院血液透析中心收治的78例MHD患者作为研究对象,采用随机抽样的方式,将MHD患者分为对照组和康复组,每组39例。两组患者均接受每周3次、每次4 h的常规血液透析治疗,康复组进行每周3次的康复运动,每次30 min,对照组不做任何康复运动。两组患者治疗前后均进行神经电生理检查并填写疲劳评定表(FAI)。比较两组患者在治疗前及治疗后8个月正中神经、尺神经、腓总神经和胫神经感觉神经传导速度(SCV)及FAI评分。结果康复组治疗后SCV、FAI评分均高于治疗前(P<0.05),康复组中各神经SCV均高于对照组(P<0.05)。治疗后,两组FAI评分均降低,且康复组低于对照组(P<0.05)。康复组正中神经、尺神经、腓总神经和胫神经有效率均为100%,对照组分别为51.3%(20/39例)、53.8%(21/39例)、69.2%(27/39例)和79.5%(31/39例),差异有统计学意义(P<0.05)。结论运动康复能改善维持性血液透析患者的周围神经功能状态及疲劳程度,提高生存质量。
Objective To objectively evaluate the functional status of peripheral nerves in patients with maintenance hemodialysis(MHD)by using neuroelectrophysiological examination,and evaluate the effects of sports rehabilitation training on peripheral nerves function and fatigue state in MHD patients.Methods A total of 78 MHD patients admitted to the Hemodialysis Center of Affiliated Hospital of Weifang Medical University from January to June 2022 were enrolled as the research objects.By random sampling,the patients were divided into control group and rehabilitation group,with 39 cases in each group.All patients received conventional hemodialysis treatment three times a week for 4 hours each time.The patients received rehabilitation exercise three times a week for 30 minutes each time in the rehabilitation group,while the patients did not receive any rehabilitation exercise in the control group.Neuroelectrophysiological examination and fatigue evaluation form(FAI)were performed before and after treatment in both groups.The sensory conduction velocity(SCV)of median nerve,ulnar nerve,common peroneal nerve and tibial nerve and FAI scores were compared between the two groups before the treatment and 8 months after the treatment.Results The SCV and FAI scores in the rehabilitation group after 8 months were significantly higher than those before the treatment(P<0.05),and the SCV in the rehabilitation group was higher than that in the control group(P<0.05).After the treatment,the FAI scores in both groups decreased,more lower in the rehabilitation group(P<0.05).The effective rates of median nerve,ulnar nerve,common peroneal nerve and tibial nerve in the rehabilitation group were 100%,while those in the control group were 51.3%(20/39 cases),53.8%(21/39 cases),69.2%(27/39 cases)and 79.5%(31/39 cases)respectively,with statistically significant(P<0.05).Conclusion Sports rehabilitation can improve the peripheral nerve function and fatigue degree of patients with MHD,and improve the quality of life.
