摘要
目的探讨Toll样受体9基因(Toll-like receptor 9,TLR9)单核苷酸多态性(single nucleotide polymorphism,SNP)与肺癌放疗患者放射性肺炎(radiation pneumonitis,RP)发生风险的关系。方法收集160例肺癌放疗患者资料,根据是否有RP,分为病例组(82例)和对照组(78例)。对所有患者TLR9基因上的3个潜在功能性SNP位点进行基因分型,检测血浆白细胞介素6(interleukin-6,IL-6)含量,分析SNP与RP发生风险和血浆IL-6含量的相关性。结果TLR9基因rs5743836位点C等位基因和TC/CC基因型与肺癌放疗患者RP发生风险增加显著相关。携带C等位基因患者放疗后RP发生风险是T等位基因的1.64倍(95%CI:1.15~2.97,P=0.013);携带rs5743836 TC和CC基因型患者放疗后RP发生风险分别是TT基因型患者的1.21倍(95%CI:1.01~1.79,P=0.044)和1.54倍(95%CI:1.12~2.48,P=0.016)。病例组血浆IL-6含量显著高于对照组(P<0.001);携带rs5743836 CC基因型患者血浆IL-6含量显著高于TT或TC型患者,TC型患者血浆IL-6含量显著高于TT型(P均<0.001)。rs5743836位点C等位基因型与血浆IL-6含量在肺癌放疗患者RP发生中存在显著的正相加交互作用(P=0.018)。结论TLR9 rs5743836位点基因型与肺癌放疗患者RP发生风险增加显著相关,可以此筛选放射性肺损伤易感人群,并早期诊断及防治放射性肺损伤。
Objective To explore the association between single nucleotide polymorphisms(SNPs)in Toll-like receptor 9(TLR9)genes and the risk of radiation pneumonitis(RP)in lung cancer patients receiving radiotherapy.Methods A total of 160 lung cancer patients receiving radiotherapy were enrolled in Air Force Hospital of Western Theater Command between August 2018 and May 2021 before they were divided into the case group(n=82)and control group(n=78)according to the occurrence of RP.Three potential functional SNPs in TLR9 genes were genotyped using the Sequenom iPLEX genotyping system,and plasma level of IL-6 in each of the patients was measured using enzyme-linked immunosorbent assay.The correlations of polymorphism genotypes with RP risk and plasma IL-6 levels were evaluated using an unconditional multivariate logistic regression model.Results The C allele and TC/CC genotypes of rs5743836 were positively correlated with the risk of RP in lung cancer patients receiving radiotherapy.Patients with rs5743836 C allele had a 1.64-fold increased risk of RP compared with patients with allele(95%CI:1.15~2.97,P=0.013).Those who carried TC genotype or CC genotype had a 1.21-fold(95%CI:1.01~1.79,P=0.044)or 1.54-fold(95%CI:1.12~2.48,P=0.016)increased risk of RP compared with patients carrying TT genotype(P=0.044 or P=0.016).Moreover,the level of plasma IL-6 in the case group was significantly higher than that of the control group(P<0.001).In both groups,the levels of plasma IL-6 detected in patients bearing rs5743836 CC genotype were significantly higher than in patients bearing TC genotype or TT genotype(P<0.001),and carriers of the rs5743836 TC genotype had higher plasma concentrations of IL-6 than those of TT genotype.In addition,TLR9 rs5743836 genotypes and plasma IL-6 levels had a positive additive effect on the occurrence of RP in lung cancer patients receiving radiotherapy(P=0.018).Conclusion Despite the small sample size,this is the first study that has suggested TLR9 rs5743836 polymorphism is associated with the risk of RP in l
