青少年腰椎间盘突出症Waveflex动态固定

Waveflex dynamic fixation for lumbar disc herniation in adolescents

摘要 [目的]评估Waveflex动态固定系统治疗青少年腰椎间盘突出症短期疗效。[方法]回顾性分析2017年6月-2020年12月采用Waveflex动态固定系统结合椎板间扩大开窗髓核摘除治疗青少年椎间盘突出23例患者的临床资料,评估其临床与影像结果。[结果]23例患者均顺利完成手术,无严重并发症,手术时间(73.8±5.5)min,术中出血量(131.7±22.4)ml,随访时间12~36个月。随时间推移(术前、术后6个月和末次随访),腰痛VAS评分[(5.4±1.0),(2.3±0.6),(1.5±0.6),P<0.001]、腿疼VAS评分[(6.5±0.9),(2.4±0.6),(1.4±0.5),P<0.001]和ODI评分[(59.0±4.8),(19.7±2.9),(13.0±3.0),P<0.001]均显著减少。影像方面,术后椎间隙高度[(8.6±1.6)mm,(10.4±1.4)mm,(10.1±1.5)mm,P<0.001]、腰椎前凸角[(18.3±8.6)°,(24.7±7.4)°,(23.9±6.4)°,P<0.001]和患椎间隙Pfirrmann分级(I/II/III/IV/V)[(0/6/15/2/0),(1/10/9/3/0),(2/13/4/4/0),P=0.009]较术前显著改善。[结论]Waveflex动态固定治疗青少年腰椎间盘突出症短期临床疗效良好,有利于维持椎间隙高度,延缓椎间盘退变。 [Objective]To evaluate the short-term clinical outcomes of Waveflex M spinal system in the treatment of lumbar disc herniation in of adolescent.[Methods]A retrospective study was conducted on 23 adolescent patients who received extended interlaminar fenestrated discectomy combined with internal fixation with WaveflexTM spinal system for intervertebral disc herniation in our hospital from June 2017 to December 2020.The clinical and imaging data were evaluated.[Results]All the 23 patients were successfully operated on without serious complications,whereas with the operation time of(73.8±5.5)min,and the intraoperative blood loss of(131.7±22.4)ml,and followed up from 12 to 36 months.As time went by(preoperative,6 months after surgery and the last follow-up),the VAS score for lumbago[(5.4±1.0),(2.3±0.6),(1.5±0.6),P<0.001],the VAS score for leg pain[(6.5±0.9),(2.4±0.6),(1.4±0.5),P<0.001],and 0DI scores[(59.0±4.8),(19.7±2.9),(13.0+3.0),P<0.001]were significantly reduced.Radiographically,the height of the intervertebral disc[(8.6±1.6)mm,(10.4±1.4)mm,(10.1±1.5)mm,P<0.001],the lumbar lordotic angle[(18.3±8.6)°,(24.7±7.4)°,(23.9±6.4)°,P<0.001],and Pfirrmann grade of disc degeneration(I/II/III/IV/V)[(0/6/15/2/0),(1/10/9/3/0),(2/13/4/4/0),P=0.009]significantly improved postoperatively compared with those preoperatively.[Conclusion]This waveflex dynamic fixation does achieve satisfactory clinical consequences in short term for adolescent lumbar disc herniation,which might be beneficial to maintain the height of intervertebral space and delay the degeneration of intervertebral disc.
