摘要
目的评估老年结直肠癌患者行回肠造口术前后的营养状况,分析影响患者营养状况变化的相关因素。方法选取2020年1月至2021年12月结直肠癌行回肠造口术的老年患者37例。应用微型营养评价(MNA)评估患者术前、术后第7、21天的营养状况。结果与术前比较,所有患者MNA评分在术后第7、21天均下降。包括体重、活动能力、体重指数(BMI)、每天正餐次数、每天摄入的水果和蔬菜的比例以及上臂中点围,差异均有统计学意义(P<0.05)。术前,33例(89.2%)患者能外出活动。术后第21天,24例(64.9%)患者能外出活动。BMI>23 kg/m^(2)的患者术前为29例(78.4%),术后第21天为11例(29.7%)。对于患者每天摄入的水果和蔬菜次数,15例(40.5%)患者术前每天至少摄入2次,而术后第21天5例(13.5%)患者每天仅1次。术前所有患者臂围均>22 cm。而在术后第21天仅13例(35.1%)患者臂围>22 cm。结论回肠造口术后前21 d,老年结直肠癌患者的营养状况因营养的摄入总量和成分减少将受到显著影响。临床医师和护士需要在这期间警惕患者的营养状况,共同监测营养参数,并在需要时提供临床治疗和护理指导。
Objective To assess the nutritional status of patients before and after the construction of the ileostomy as well as nutrition-related factors.Methods Thirty-seven elderly patients who underwent ileostomy for colorectal cancer hospitalized in Huzhou Central Hospital from January 2020 to December 2021 were selected.The evaluation tool used was"Mini Nutritional Assessment(MNA)".Patients underwent nutritional assessment before the surgery(time 0),on the 7th post-operative day(time 1),and on the 21th post-operative day(time 2).Results All patients had a drop in MNA score on the 7th and 21th post-operative days.Factors associated with MNA were weight loss,mobility,body mass index(BMI),number of full meals consumed per day,portions of fruits and vegetables consumed per day,and mid-arm circumference,(P<0.05,respectively).Pre-operatively,33(89.2%)of participants were able to move on their own and 24(64.9%)on the 21th post-operatively day.Furthermore,BMI>23 kg/m^(2)had 29(78.4%)of participants pre-operatively and 11(29.7%)on the 21th post-operative day.In terms of portions of fruits and vegetables consumed per day,15(40.5%)of patients consumed at least 2 times,pre-operatively and 5(13.5%)of patients consumed only 1 time on the 21th post-operatively day.Moreover,pre-operatively all participants(100%)had arm circumference>22 cm while on the 21th post-operative day,only 13(35.1%)of participants had arm circumference>22 cm.Conclusion In the first 21 days after the construction of an ileostomy,the nutritional status of the elderly patients with colorectal cancer is significantly affected.Clinicians and nurses need to be alert to the nutritional status of patients during this period,monitor nutritional parameters together,and provide clinical treatment and nursing guidance when necessary.
