利用扫描式视网膜厚度分析仪分析黄斑裂孔

摘要 目的 利用扫描式视网膜厚度仪探讨黄斑裂孔的组织学改变。方法 利用视网膜厚度分析仪 (RTA )扫描 2 2例 (2 3只眼 )临床明确或可疑黄斑裂孔眼的后极部视网膜。在 0 .2~ 0 .4秒内得到多个视网膜切面 ,进而判断临床诊断的准确性 ,了解黄斑区视网膜内部及表面的改变。结果 板层及全层黄斑裂孔在 RTA检查下表现为内表面破裂的视网膜内空腔样改变。板层裂孔的空腔底部有特征性的组织残留。 15只眼裂孔周围视网膜明显增厚 ,5只眼 (2 1.7% )在 RTA下的诊断结果与临床不符。 8只眼伴有视网膜表面膜 ,均伴有黄斑裂孔和视网膜内囊肿样改变。结论 通过 RTA仪能准确诊断黄斑裂孔 。 Objective This nonconsecutive case series study aimed to study the structural changes associated with clinical diagnosed macular hole.Methods 23 eyes of 22 patients who was clinically suspeced or confirmed macular hole were included in this study.A scanning retinal thickness analyzer(RTA) was used to scan the posterior retinal of them.It yielded multiple optical cross section of retina in 0.2 to 0.4 seconds,which helps to determine the accuracy of clinical diagnoses and understand the intraretinal and epiretinal changes of these macular regions.Results Partial-thickness and full-thickness macular holes correspond to intraretinal cavities in which the inner retinal surface was broken.Some tissue left at the bottom of the cavities was found characteristically in partial-thickness macular holes.15 eyes showed significant retinal thickness increase surrounding the hole.The RTA diagnoses of 5 eyes(21.7%) were different from the clinical diagnoses.Epiretinal membrane was found in 8 eyes,which all combined with macular hole and intraretinal cyst.Conclusions The scanning RTA provides useful and accurate information in the diagnosis of macular holes.It confirms the hypothesis that idiopathic macular holes come from local vitreoretinal traction and intraretinal cyst.
