摘要
目的:探讨阴道术后梭形细胞结节(PSCN)的临床特点、病理形态学、免疫表型、荧光原位杂交(FISH)检测、诊断、鉴别诊断和预后。方法:收集4例阴道PSCN进行临床表现、形态学特征、免疫组化及FISH检测的观察和分析,并复习文献。结果:4例阴道PSCN均有病灶区域近期手术病史。光镜下见梭形细胞增生伴较多核分裂象,但缺乏核多形性以及病理性核分裂象。免疫组化显示CK在2例中局灶弱阳性,Vimentin、SMA(电车轨道染色模式)在4例中均弥漫强阳性,ALK(D5F3)均阴性。FISH检测显示4例USP6基因断裂均为阳性。结论:USP6基因重排首次在阴道PSCN中被发现,这有助于PSCN的诊断。阴道PSCN的一个子集具有USP6基因重排,鉴于其与结节性筋膜炎(NF)具有相似性,故可以将PSCN归类为USP6相关的纤维母细胞/肌纤维母细胞性肿瘤(UAN)。
Objective:To investigate the clinical features,pathological morphology,immunophenotype,fluorescence in situ hybridization(FISH)detection,diagnosis,differential diagnosis and prognosis of postoperative spindle cell nodule(PSCN)in vagina.Methods:The clinical manifestations,morphological features,immunohistochemistry and FISH detection of 4 cases of vaginal PSCN were analyzed,and the literature was reviewed.Results:All the 4 cases of vaginal PSCN had a history of recent operation in the diseased area.Light microscopy showed spindle cell proliferation with more mitosis,but lack of nuclear pleomorphism and pathological mitosis.Immunohistochemistry staining showed that CK was focally and weakly positive in 2 cases,Vimentin and SMA(tram track staining pattern)were diffusely and strongly positive in 4 cases,and ALK(D5F3)was negative in 4 cases.FISH test showed that all 4 cases were positive for USP6 gene rearrangement.Conclusion:USP6 gene rearrangement was found in vaginal PSCN for the first time,which is helpful for the diagnosis of PSCN.A subset of vaginal PSCN has USP6 gene rearrangement,since its similarity to nodular fasciitis(NF),PSCN could be classified as USP6-associated fibroblastic/myofibroblastic tumor(UAN).
