摘要
目的:基于模块化分类管理理念构建分级管理模式,提升腔镜手术器械临床应用质量。方法:根据腔镜手术器械临床使用情况将其分为通用型、专用型和特定型,制定分段管理、联合管理和专项管理相结合的管理方法,形成模块化分类管理模式。选取医院收治的400例腔镜手术患者和对应的400套次腔镜手术器械包,按照腔镜手术器械管理模式不同将其分为对照组和观察组,每组各200例患者和对应的200套次器械包,对照组腔镜手术器械采用常规化统一管理模式,观察组采用模块化分类管理模式,对比两组器械质量、诊疗效果进行比较,自制调研问卷,对器械服务水平进行比较。结果:观察组器械部件损坏率、备件差错率和使用故障率分别为0.5%(1/200)、3.0%(3/200)和0.0%(0/200),均低于对照组,消毒合格率为99.0%(198/200),高于对照组,组间差异有统计学意义(χ^(2)=15.969,χ^(2)=12.623,χ^(2)=5.063,χ^(2)=4.581;P<0.05);观察组器械在普通外科、泌尿外科、妇科和胸外科使用中的相关感染风险率分别为4.00%(2/50)、4.41%(3/68)、2.17%(1/46)和0.00%(0/36),均低于对照组,组间差异有统计学意义(χ^(2)=4.288,χ^(2)=5.234,χ^(2)=4.148,χ^(2)=7.169;P<0.05);参与调研的手术科室医护人员对观察组腔镜使用便捷性、疾病治疗效果和临床安全性满意度评分分别为(18.89±0.93)分、(27.94±1.97)分和(19.01±1.02)分,均高于对照组,组间差异有统计学意义(t=4.372,t=5.592,t=3.903;P<0.05)。结论:模块化分类管理模式能够提高医护人员使用腔镜手术器械的规范性,降低腔镜手术器械的安全风险和感染风险发生率,提升临床诊疗服务水平。
Objective:To construct a hierarchical management model based on the concept of modular classification management and to improve the clinical application quality of endoscopic surgical instruments.Methods:According to the clinical use of endoscopic surgical instruments,the endoscopic surgical instruments were divided into general type,special type and specific type,and a management method combining segmented management,joint management and special management was developed to form a modular classification management mode.A total of 400 patients with endoscopic surgery and the corresponding 400 sets of endoscopic surgery instruments were selected and divided into control group and observation group according to different management modes of endoscopic surgery instruments,with 200 patients and the corresponding 200 sets of instruments in each group.The control group adopted the conventional unified management mode for endoscopic surgery instruments,and the observation group adopted the modular classification management mode.The difference of instrument quality,diagnosis and treatment effects and service level between the two groups was compared.Results:The instrument component damage rate,spare parts error rate and service failure rate of the observation group were 0.5%,3.0%and 0.0%,respectively,which were lower than those of the control group,the qualified rate of disinfection was 99.0%,which was higher than that of the control group,the difference between groups was statistically significant(χ^(2)=15.969,χ^(2)=12.623,χ^(2)=5.063,χ^(2)=4.581;P<0.05).The risk rates of device-associated infection of general surgery,urology,gynecology and thoracic surgery in the observation group were 4.00%,4.41%,2.17%and 0.00%,respectively,which were lower than those in the control group,the difference between the groups was statistically significant(χ^(2)=4.288,χ^(2)=5.234,χ^(2)=4.148,χ^(2)=7.169;P<0.05).The satisfaction of medical staff in the surgical department on the convenience of endoscopic use,the effect of disease tre
