摘要
目的:探讨超声乳腺影像报告和数据系统(BI-RADS)对乳腺硬化性腺病(SA)背景下乳腺癌的诊断价值。方法:选取医院收治的60例女性SA患者,均行超声检查及BI-RADS分类,观察并记录患者病变的大小、形态、边界、内部回声以及肿块血流分布情况,以病理结果为“金标准”,分析SA背景下良恶性病变的超声图像特征差异,分析BI-RADS不同分类对诊断SA背景下乳腺恶性病变的诊断效能。结果:SA背景下乳腺癌病变与SA背景下良性病变超声表现的纵横比差异有统计学意义(χ^(2)=16.461,P<0.05),边界、形态、内部回声、内部钙化及血流均无统计学意义。受试者工作特征(ROC)曲线分析显示,BI-RADS诊断SA背景下乳腺癌病变的ROC曲线下面积(AUC)为0.96。结论:超声检查及BI-RADS分类在临床中有助于鉴别SA背景下乳腺癌,且操作简便,安全无创,具有较高的临床应用价值。
Objective:To explore the diagnostic value of ultrasound combined with breast imaging reporting and data system(BI-RADS)for breast cancer under the background of sclerosing adenosis(SA)of breast.Methods:A total of 60 female SA patients admitted to the hospital were selected,and all of them underwent ultrasound examination and BIRADS classification.The size of lesion,morphology,boundary,internal echo and the distribution of blood flow of breast mass were observed and recorded.The results of pathological examination were used as the"gold standard".And then,the differences of the characteristics of the ultrasound images between benign and malignant lesions under SA background were analyzed.The diagnostic efficiencies of the different BI-RADS classification in diagnosing the malignant lesions of breast under the SA background were further analyzed.Results:The difference of the aspect ratio between the lesion of breast cancer and the benign lesion under SA background was significant(χ^(2)=16.461,P<0.05),and there were no significant differences in the boundary,morphology,internal echo,internal calcifications and blood flow between them(P>0.05).The analysis of receiver operating characteristic(ROC)curve showed that the area under curve(AUC)of BIRADS in diagnosing the lesion of breast cancer under SA background was 0.96.Conclusion:Ultrasound examination combined with BI-RADS classification are helpful to identify breast cancer under SA background,which operation is simple,and which is safety and noninvasiveness.It has higher clinical application value.
