先天性马蹄内翻足患儿经Ponseti法治疗后在学龄期MRI表现的初步研究

摘要 目的研究先天性马蹄内翻足患儿经过Ponseti法治疗后在学龄期的距骨、舟骨、距舟关节的形态。方法采集中国医科大学附属盛京医院2013年7月至2018年8月收治的6例(7足)学龄期先天性马蹄内翻足患儿的MRI影像,采用三维重叠数字减影技术,分析距骨、跟骨形态和距舟关节的关系。结果7足的距骨颈角平均为30.99°,舟骨角为38.04°,舟骨水平覆盖率为97.46%,跟骨水平内翻角为81.8°,距跟角为16.74°,舟骨矢状覆盖率为94.72%,跟骨冠状内翻角为12.82°。5例对照足的距骨颈角平均为21.57°,舟骨角为25.61°,舟骨水平覆盖率为88.42%,跟骨水平内翻角为86.57°,距跟角为33.55°,舟骨矢状覆盖率为90.68%,跟骨冠状内翻角为6.53°。结论先天性马蹄内翻足患儿的距骨颈角、舟骨角、跟骨内翻角较对照足大,距跟角较对照足小,距舟覆盖率较对照足大。MRI可以很好地评估学龄期先天性马蹄内翻足患儿距骨、跟骨和距舟关节的形态。 Objective To study the morphology of the talus,scaphoid bone,and taloschoid joint in children with congenital talipes equinovarus treated with Ponseti method at school age.Methods MRI images of 6 school-age children(7 feet)were collected,and threedimensional overlapping digital subtraction technology was used to analyze the relationship between the morphology of the talus and calcaneus,and the talus scaphoid joint.Results The average talus neck angle of 7 feet was 30.99°,scaphoid angle was 38.04°,and the horizontal coverage rate of the scaphoid was 97.46%.The calcaneal horizontal varus angle was 81.8°,and the distance from the calcaneal angle was 16.74°.The sagittal coverage rate of the scaphoid was 94.72%,and the calcaneal coronal varus angle was 12.82°.The average talus neck angle and scaphoid angle of the 5 control feet were 21.57°and 25.61°,with a horizontal coverage rate of 88.42%for the scaphoid bone,86.57°for the calcaneal bone level varus angle,33.55°for the calcaneal angle,90.68%for the scaphoid bone sagittal coverage rate,and 6.53°for the calcaneal bone coronal varus angle.Conclusion Children with congenital talipes equinovarus have larger talus neck angle,scaphoid angle,and calcaneal varus angle compared to the control foot,smaller talus heel angle,and larger talus navicular coverage compared to the control foot.MRI can effectively evaluate the morphology of the talus,calcaneus,and talosacral joint in school-age children.
