摘要
目的探讨血小板参数、红细胞参数与早产儿视网膜病变(ROP)之间的相关性.方法选择在本院检查发现的ROP患者59例118眼作为ROP组,同时选择相同出生胎龄段无ROP早产儿作为非ROP早产儿组,共22例44眼.ROP组患儿根据眼底检查结果分为1/2期ROP组、阈值期/阈值前病变ROP组、A-ROP组.收集所有研究对象的出生孕周、出生体重等临床资料,采集肘部静脉血检测血象各指标,比较血小板计数(PLT)、平均血小板体积(MPV)、红细胞计数(RBC)、红细胞分布宽度(RDW)等指标.使用logistic回归分析ROP的危险因素.结果ROP组、非ROP早产儿组之间的出生体重、呼吸窘迫、机械通气相比差异有统计学意义(P<0.05);1/2期ROP组、阈值期/阈值前病变ROP组、A-ROP组患儿的出生体重、呼吸窘迫、机械通气比较差异有统计学意义(P<0.05).ROP组RBC、PLT、MPV、RDW与非ROP早产儿组相比差异有统计学意义(P<0.05);1/2期ROP组、阈值期/阈值前期ROP组、A-ROP组患儿的RBC、PLT比较差异有统计学意义(P<0.05).多因素logisitc回归分析,结果显示:PLT是ROP发生的危险因素(P<0.05),而PLT、RBC是影响ROP病情的严重的危险因素(P<0.05).结论出生体重、机械通气、PLT、RBC、MPV、RDW与ROP发生发展有较大关系,PLT是ROP发生的危险因素,同时也是影响ROP病情严重程度的危险因素,可对临床上认识ROP病情提供参考.
Objective To investigate the correlation between platelet parameters,red blood cell parameters,and retinopathy of prematurity(ROP).Methods 59 patients(118 eyes)with ROP were selected as ROP group,and 22 premature infants(44 eyes)without ROP,and 22 premature infants without ROP in the same gestational age range were selected as the non-ROP group.The ROP group was divided into three subgroups according to the results of fundus examination:stage 1/2 ROP,threshold/pre-threshold ROP,and aggressive posterior ROP(A-ROP).Clinical data such as gestational age and birth weight were collected for all study subjects,and blood samples were collected from the elbow vein to measure platelet count(PLT),mean platelet volume(MPV),red blood cell count(RBC),red blood cell distribution width(RDW),and other parameters.Logistic regression analysis was used to identify risk factors for ROP.Results There were significant differences in birth weight,respiratory distress,and mechanical ventilation between the ROP and non-ROP premature infant groups(P<0.05).The same parameters also showed significant differences among the three ROP subgroups(P<0.05).RBC,PLT,MPV,and RDW showed significant differences between the ROP and non-ROP premature infant groups(P<0.05),and PLT and RBC were identified as significant risk factors for ROP and its severity(P<0.05)based on multiple factor logistic regression analysis.Conclusion Birth weight,mechanical ventilation,PLT,RBC,MPV,and RDW are closely related to the occurrence and development of ROP.PLT is a risk factor for ROP and also affects the severity of the disease,which can provide reference for clinical understanding of ROP.
