摘要
目的:探讨影响膀胱癌尿脱落细胞学检查结果的临床相关因素。方法:选择2013年12月至2017年12月行术前尿脱落细胞学检查的膀胱癌患者1017例,比较尿脱落细胞学检查阳性与阴性膀胱癌患者的临床特征。采用Logistic回归分析膀胱癌尿脱落细胞学检查结果的影响因素。结果:1017例患者尿脱落细胞学检查阳性率为58.8%。尿脱落细胞学检查阳性与阴性患者的性别、年龄,肿瘤病理分期、病理分级和标本送检次数比较,差异有统计学意义(P<0.05)。Logistic回归分析结果显示性别(OR=2.394,95%CI为1.689~3.394)、年龄(OR=1.036,95%CI为1.023~1.049)、术前有无肉眼血尿(OR=1.379,95%CI为1.024~1.855)、肿瘤病理分级(OR=2.755,95%CI为2.061~3.683)以及标本送检次数(OR=1.900,95%CI为1.068~3.377)是膀胱癌患者术前尿脱落细胞学检查阳性的关联因素。结论:膀胱癌患者的性别、年龄、术前有无肉眼血尿、肿瘤病理分级和样本送检次数与尿脱落细胞学检查结果有关联。
Aim:To investigate the clinically relevant factors affecting the results of urinary cytology in patients with bladder cancer.Methods:A total of 1017 patients who underwent surgery at the Second Hospital of Tianjin Medical University were chosen.Clinical characteristics of patients with positive or negative urinary cytology were compared.The relevant factors affecting the results of urinary cytology were analyzed using Logistic regression.Results:The overall positive rate of urinary cytology was 58.8%.There were significant differences between the 2 groups in terms of gender,age,pathological stage,pathological grading,and number of samples(P<0.05).Logistic regression analysis revealed that gender(OR=2.394,95%CI was 1.689-3.394),age(OR=1.036,95%CI was 1.023-1.049),gross hematuria before surgery(OR=1.379,95%CI was 1.024-1.855),pathological grading(OR=2.755,95%CI was 2.061-3.683)and the number of samples(OR=1.900,95%CI was 1.068-3.377)were relevant factors affecting the results of urinary cytology in patients with bladder tumor.Conclusions:Gender,age,gross hematuria before surgery,pathological grading,and number of samples may be associated with the results of urinary cytology.
