摘要
目的探究薏苡附子败酱散加味汤联合抗生素治疗慢性子宫内膜炎(CE)的疗效及对患者炎症反应的影响。方法70例气虚血瘀兼湿热型CE患者按随机数字表法分为中西组和西药组。均予多西环素治疗,中西组加予薏苡附子败酱散加味汤,比较两组临床疗效、中医症状积分、子宫内膜指标[子宫内膜厚度、血流指数(FI)、阻力指数(RI)]、炎症反应[肿瘤坏死因子-α(TNF-α)、白介素-10(IL-10)、白介素-1β(IL-1β)]、子宫内膜CD38及CD138阳性率、不良反应。结果治疗后,中西组临床有效率高于西药组(P<0.05);治疗后,两组中医症状积分均下降(P<0.05),子宫内膜厚度、FI均升高(P<0.05),RI均下降(P<0.05),血清TNF-α、IL-1β水平均下降(P<0.05),IL-10水平均升高(P<0.05),子宫内膜CD38、CD138阳性例数均下降(P<0.05),且中西组治疗后上述指标与西药组比较,差异具有统计学意义(P<0.05);两组治疗期间均未见明显不良反应。结论薏苡附子败酱散加味汤联合抗生素可提高气虚血瘀兼湿热型CE的临床疗效,抑制炎症反应,改善子宫内膜环境。
Objective To explore the efficacy of modified Yiyi Fuzi Baijiang powder combined with antibiotics in the treatment of chronic endometritis(CE)and its effects on inflammatory response.Methods 70 patients with CE of qi deficiency,blood stasis and dampness-heat were divided into Chinese-western medicine group and western medicine group according to the random number table method.All patients were treated with doxycycline,and the Chinese-western medicine group was additionally given modified Yiyi Fuzi Baijiang powder.Clinical efficacy,traditional Chinese medicine(TCM)symptoms scores,endometrial indicators[endome trialthickness,blood flow index(FI),resistance index(RI)],inflammatory response[tumor necrosis factor-α(TNF-α),interleukin-10(IL-10),interleukin-1β(IL-1β)],positive rates of endometrial CD38 and CD138 and adverse reactions were compared between the two groups.Results After treatment,the clinical effective rate in Chinese-western medicine group was higher than that in western medicine group(P<0.05).After treatment,the scores of TCM symptoms in both groups were decreased(P<0.05),and the endometrial thickness and FI were enhanced(P<0.05)while the RI was reduced(P<0.05),and the levels of serum TNF-αand IL-1βwere declined(P<0.05)while the level of IL-10 was elevated(P<0.05),and the cases with positive endometrial CD38 and CD138 were decreased(P<0.05),and there were statistically significant differences in the above indicators in Chinese-western medicine group compared with those in western medicine group after treatment(P<0.05).No obvious adverse reactions were observed in the two groups during treatment.Conclusions Modified Yiyi Fuzi Baijiang powder combined with antibiotics can enhance the clinical efficacy of CE with qi deficiency,blood stasis and dampness-heat,inhibit the inflammatory response,and improve the endometrial environment.
