摘要
目的 探讨一期患侧朝上斜仰截石位与分期截石位输尿管软镜治疗肾结石的疗效。方法 方便选择2020年5月—2021年10月于龙岩市第一医院接受治疗的54例肾结石患者为对象,根据手术方式不同分为一期斜仰截石位手术组(n=32)和分期截石位手术组(n=22)。一期斜仰截石位手术组予以一期患侧朝上斜仰截石位输尿管软镜钬激光碎石术(flexible ureteroscopic lithotripsy,FURL)治疗,分期截石位手术组给予分期截石位FURL治疗,治疗后评估两组临床疗效,比较两组手术时间、住院时间、结石清除率及术后并发症发生情况。结果 两组患者手术均顺利完成,并未出现严重并发症。两组手术时间比较,差异无统计学意义(P>0.05)。两组住院时间比较,差异有统计学意义(P<0.05)。一期斜仰截石位手术组结石清除率(93.75%)高于分期截石位手术组(68.18%),一期斜仰截石位手术组并发症发生率(3.13%)显著低于分期截石位手术组(27.27%),差异有统计学意义(χ^(2)=4.434、4.768,P<0.05)。结论 临床治疗肾结石采用一期患侧朝上斜仰截石位输尿管软镜钬激光碎石术治疗效果优于分期截石位输尿管软镜治疗效果,患者结石清除率更高,且一期手术的并发症发生率较低。
Objective To investigate the efficacy of the treatment of renal calculi by phase I upward oblique supine lithotomy position versus staged lithotomy ureteroscopy.Methods Fifty-four patients with kidney stones who received treatment in the First Hospital of Longyan City from May 2020 to October 2021 were conveniently selected as subjects,and divided into the first-stage oblique lithotomy operation group(n=32)and the stage-stage lithotomy operation group(n=22)according to different surgical methods.The first-stage oblique lithotripsy operation group was treated with first-stage flexible ureteroscopic holmium laser lithotripsy(FURL)with the affected side facing upward oblique lithotripsy,and the staged lithotripsy operation group was treated with FURL at the staged lithotripsy.The clinical efficacy of the two groups was evaluated after treatment.Operation time,hospital stay,stone clearance rate and postoperative complications were compared between the two groups.Results The operation was successfully completed in both groups without serious complications.There was no significant difference in operation time between the two groups(P>0.05).Comparison of hospitalization time between two groups,and the difference was statistically significant(P<0.05).The stone clearance rate of the first stage oblique lithotomy operation group was 93.75%,which was higher than that of the stage lithotomy operation group(68.18%),the complication rate of the first stage oblique lithotomy operation group(3.13%)was significantly lower than that of the stage lithotomy operation group(27.27%),and the difference was statistically significant(χ^(2)=4.434,4.768,P<0.05).Conclusion In the clinical treatment of renal calculi,first-stage ureteroscopic holmium laser lithotripsy with the affected side up oblique lithotripsy was better than that with the stage lithotripsy soft ureteroscope,with higher stone clearance rate and lower complication rate of firststage surgery.
