摘要
目的 分析腰硬联合麻醉对老年高血压患者麻醉诱导期心率变异性的影响。方法 随机选取2020年1月—2021年1月期间在厦门市第五医院进行手术的90例老年患者,依据患者血压情况分为高血压组、血压正常组,各45例,所有患者均进行腰硬联合麻醉,比较两组患者不同时间点心率、血压指标变化,波动幅度。结果 麻醉前,两组心率比较差异无统计学意义(P>0.05);麻醉5、10、30 min后,高血压组心率明显低于血压正常组,差异有统计学意义(P<0.05)。麻醉后5 min血压正常组舒张压为(78.96±4.31)mmHg低于高血压组的(82.56±4.07)mmHg,差异有统计学意义(t=4.074,P<0.05)。麻醉前及麻醉后5 min,高血压组收缩压均高于血压正常组,差异有统计学意义(P<0.05)。两组麻醉前及麻醉5、10、30 min后低频、高频对比差异有统计学意义(P<0.05),且两组麻醉5、10、30 min低频、高频均低于术前,差异有统计学意义(P<0.05)。结论 腰硬联合麻醉对老年高血压患者麻醉诱导期的心率、血压均会产生不同程度的影响,因此临床在对老年患者实施麻醉手术时,应监测患者各项指标变化情况,预防不良事件发生。
Objective To analyze the effect of combined lumbar and rigid anesthesia on heart rate variability during the induction period of anesthesia in elderly hypertensive patients.Methods 60 elderly patients who underwent surgery in the Fifth Hospital of Xiamen between January 2020 and January 2021 were selected randomly.According to the patients' blood pressure,they were divided into hypertensive group and normal blood pressure group,45 cases in each,and all patients underwent combined lumbar and rigid anesthesia,comparing the changes of heart rate and blood pressure indexes and the magnitude of fluctuations in the two groups at different times.Results Before anesthesia,there was no statistically significant difference in heart rate between the two groups(P>0.05.After 5min,10min,and 30 min of anesthesia,the heart rate of the hypertensive group was significantly lower than that of the normotensive group,the difference was statistically significant(P<0.05).After 5 minutes of anesthesia,the diastolic blood pressure of the normal blood pressure group was(78.96±4.31)mmHg,which was lower than that of the hypertension group(82.56±4.07)mmHg,the difference was statistically significant(t=4.074,P<0.05).Before and 5 minutes after anesthesia,the systolic blood pressure in the hypertensive group was higher than that in the normotensive group,the difference was statistically significant(P<0.05).There was a statistically significant difference in low frequency and high frequency between the two groups before anesthesia,5 min,10 min and 30 min after anesthesia(P<0.05),and the low frequency and high frequency between the two groups at 5 min,10 min and 30 min of anesthesia were lower than before surgery,the difference was statistically significant(P<0.05).Conclusion Combined lumbar and rigid anesthesia has different effects on heart rate and blood pressure during the induction of anesthesia in elderly hypertensive patients.Therefore,the clinical implementation of anesthesia surgery on elderly patients should monitor the changes in the
