摘要
目的 探讨鼻中隔三线减张法矫正术在鼻内窥镜鼻窦手术治疗慢性鼻炎并鼻中隔偏曲患者中的应用效果。方法 方便选择2020年4月—2021年3月菏泽市第二人民医院收治的慢性鼻炎并鼻中隔偏曲患者88例,以随机数表法分为两组,各44例。对照组行鼻内窥镜鼻窦手术治疗,研究组在对照组基础上联合鼻中隔三线减张法矫正术。针对两组的临床疗效、术前与术后患者的临床症状评分与视觉模拟评分(VAS),以及术后并发症情况进行观察与对比。结果 研究组临床治疗总有效率为97.72%,较对照组(81.82%)高,差异有统计学意义(χ^(2)=4.456,P<0.05)。研究组术后临床症状评分中,鼻分泌物(0.88±0.42)分、鼻塞(0.92±0.44)分、头晕头痛(1.02±0.35)分、嗅觉障碍(0.67±0.52)分较对照组(2.06±0.39)分、(2.02±0.23)分、(1.55±0.47)分、(1.88±0.69)分低,差异有统计学意义(t=13.657,14.696,5.999,9.290,P<0.05)。术后3周时研究组VAS评分(1.65±0.52)分较对照组(3.00±0.47)分低,差异有统计学意义(t=12.776,P<0.05)。两组术后并发症发生率均为4.55%,差异无统计学意义(χ^(2)=0.000,P>0.05)。结论 在慢性鼻炎并鼻中隔偏曲患者的鼻中隔偏曲鼻内窥镜鼻窦手术中应用鼻中隔三线减张法矫正术治疗可以取得满意的效果,适于临床推广。
Objective To investigate the application effect of nasal septum three-line reduction method in endoscopic sinus surgery in the treatment of chronic rhinitis with deviated nasal septum.Methods A total of 88 patients with chronic rhinitis and deviated nasal septum admitted to Heze Second People's Hospital from April 2020 to March 2021 were convenient selected and divided into two groups by random number table method,44 patients in each group.The control group were treated with nasal endoscopic sinus surgery,and the study group were combined with three-line nasal septum reduction method on the basis of the control group.The clinical curative effect,preoperative and postoperative clinical symptom scores and visual analogue scale(VAS)of the two groups,as well as postoperative complications were observed and compared.Results The total effective rate of the study group was 97.72%higher than that of the control group 81.82%,and the difference was statistically significant(χ^(2)=4.456,P<0.05).In the postoperative clinical symptom score of the study group,nasal secretion(0.88±0.42)points,nasal congestion(0.92±0.44)points,dizziness and headache(1.02±0.35)points,and olfactory disturbance(0.67±0.52)points were lower than those in the control group(2.06±0.39)points,(2.02±0.23)points,(1.55±0.47)points and(1.88±0.69)points,and the difference was statistically significant(t=13.657,14.696,5.999,9.290,P<0.05).At 3 weeks after operation,the VAS score of the study group(1.65±0.52)was lower than that of the control group(3.00±0.47)points,and the difference was statistically significant(t=12.776,P<0.05).In the comparison of the incidence of postoperative complications,both groups were 4.55%,and the difference was no statistically significant(χ^(2)=0.000,P>0.05).Conclusion For patients with chronic rhinitis and nasal septum deviation,the three-line nasal septum reduction method can achieve satisfactory results in endoscopic sinus surgery for nasal septum deviation,which is suitable for clinical promotion.
