摘要
目的分析耳内镜手术治疗慢性化脓性中耳炎的临床效果。方法选择2019年2月—2022年11月沛县人民医院收治的46例慢性化脓性中耳炎患者为研究对象,以随机数表法分为对照组(23例,显微镜下鼓室修补术治疗)、观察组(23例,耳内镜下鼓室修补术治疗)。对比两组手术指标、治疗效果、并发症及听阈改善情况。结果观察组出血量、手术时间、术后24 hVAS评分分别为(6.31±1.25)mL、(71.56±3.24)min、(2.26±0.75)分,均优于对照组的(19.56±2.34)mL、(101.25±4.62)min、(3.65±0.78)分,差异有统计学意义(t=23.953、25.233、6.161,P<0.05)。疗效对比,观察组95.65%优于对照组的65.21%,差异有统计学意义(χ^(2)=4.973,P<0.05)。观察组并发症发生率4.35%低于对照组的39.13%,差异有统计学意义(χ^(2)=8.178,P<0.05)。治疗后,观察组气导听阈分值、骨导听阈分值均低于对照组,差异有统计学意义(t=5.536、4.126,P<0.05)。结论耳内镜手术治疗慢性化脓性中耳炎效果显著、手术时间短、出血少、疼痛轻微、患者术后恢复快、听力有明显改善,且手术安全性高,无严重并发症。
Objective To analyze the clinical effect of endoscopic surgery on chronic suppurative otitis media.Methods From February 2019 to November 202246 patients with chronic suppurative otitis media admitted to Peixian People's Hospital were selected as the study subjects.They were randomly divided into control group(23 cases,treated with tympanoplasty under microscope)and observation group(23 cases,treated with tympanoplasty under otoscope).The operative indexes,therapeutic effect,complications and improvement of hearing threshold were compared between the two groups.Results The bleeding volume,operation time and 24 h VAS score of the observation group were(6.31±1.25)mL,(71.56±3.24)min and(2.26±0.75)points respectively,which were better than than those of the control group(19.56±2.34)mL,(101.25±4.62)min and(3.65±0.78)points,the difference was statistically significant(t=23.953,25.233,6.161,P<0.05).The curative effect of the observation group 95.65%was better than that of the control group 65.21%,the difference was statistically significant(χ^(2)=4.973,P<0.05).The incidence of complications in the observation group 4.35%was lower than that in the control group 39.13%,the difference was statistically significant(χ^(2)=8.178,P<0.05).After treatment,the score of air conduction hearing threshold and bone conduction hearing threshold in the observation group were lower than those in the control group,the difference was statistically significant(t=5.536,4.126,P<0.05).Conclusion Endoscopic surgery is effective in treating chronic suppurative otitis media,with short operation time,less bleeding,mild pain,rapid postoperative recovery,significant improvement in hearing,high safety and no serious complications.
