摘要
目的探讨早期肠内营养支持对肝癌患者术后胃肠功能恢复和营养状态的影响。方法选择2021年6月至2022年6月于丽水市中心医院行腹腔镜肝癌根治术的81例患者,依据随机数字表法分为观察组41例与对照组40例。观察组采用早期肠内营养支持治疗,对照组采用全肠外营养支持治疗。两组治疗周期均为7 d。比较两组患者术后胃肠功能恢复时间;两组术前、术后营养状况指标、血清炎性因子水平和肝功能变化。结果观察组术后进流质时间、排气时间和排便时间均显著短于对照组(均P<0.05)。两组术后转铁蛋白、白蛋白和前白蛋白水平均显著低于术前(均P<0.05);相比对照组,观察组术后转铁蛋白、白蛋白和前白蛋白水平均显著升高(均P<0.05)。两组术后血清CRP、IL-6和TNF-α水平均显著高于术前(均P<0.05),相比对照组,观察组术后血清CRP、IL-6和TNF-α水平均显著降低(均P<0.05)。两组术后ALT和AST水平均显著高于术前(均P<0.05),相比对照组,观察组AST和ALT水平均显著降低(均P<0.05)。结论肝癌术后患者采用早期肠内营养支持治疗可加快胃肠功能恢复,营养状态好转,炎性反应减轻,肝功能得到改善。
Objective To explore the effect of early enteral nutrition support on the recovery of gastrointestinal function and nutritional status in patients with liver cancer after surgery.Methods Eighty-one patients with liver cancer who underwent laparoscopic radical resection in Lishui Central Hospital from June 2021 to June 2022 were randomly divided into two groups,patients in study group(41 cases)received early enteral nutrition,while patients in control group(40 cases)receive total parenteral nutrition support,the course was for 7 days.The nutritional status,serum inflammatory factor levels and liver function were all measured before and after surgery in two groups;the time of gastrointestinal function recovery was compared between two groups.Results The postoperative fluid intake time,exhaust time,and defecation time in the study group were all significantly shorter than those in control group(all P<0.05).The serum levels of transferrin,albumin and prealbumin after surgery were significantly decreased compared to those before surgery in both groups(all P<0.05);however,the levels in the study group were significantly higher than those in control group(all P<0.05).The serum CRP,IL-6 and TNF-α levels were all significantly increased compared to those before surgery in two groups(all P<0.05);however,the levels in study group were significantly lower than those in the control group(all P<0.05).The serum AST and ALT levels were both significantly increased after surgery compared to those before surgery in both groups(both P<0.05);however,the levels in study group were significantly lower than those in the control group(both P<0.05).Conclusion Early enteral nutrition support treatment for postoperative patients with liver cancer can accelerate the recovery of gastrointestinal function,improve nutritional status,alleviate inflammatory reactions,and improve liver function.
