摘要
目的探究高频超声联合彩色多普勒超声对甲状腺良恶性结节的诊断价值。方法以2020年11月-2022年5月天津市环湖医院收治的200例甲状腺结节患者为研究对象,其病理学结果显示甲状腺良性结节132例,甲状腺恶性结节68例。所有患者均接受高频超声与彩色多普勒超声诊断,分析其声像图特征及血流参数[颈动脉收缩期最大流速(PSV)及血管阻力指数(RI)],比较高频超声、彩色多普勒超声与联合诊断对甲状腺性良、恶性结节的诊断效能。结果甲状腺良、恶性结节的高频超声声像图特征比较(结节结构、结节回声、结节形态、结节边缘、结节钙化灶),差异有统计学意义(P<0.05);甲状腺良、恶性结节的彩色多普勒血流特征比较(PSV、RI),差异有统计学意义(P<0.05);联合诊断甲状腺性良恶性结节的诊断准确性、敏感性、特异性高于高频超声与彩色多普勒超声,差异有统计学意义(P<0.05)。结论高频超声联合彩色多普勒超声在甲状腺良、恶性结节的诊断中具有较高准确性、敏感性及特异性,其诊断价值高于单一超声检测。
Objective To explore the diagnostic value of high frequency ultrasound combined with color Doppler ultrasound in benign and malignant thyroid nodules.Methods A total of 200 patients with thyroid nodules admitted to Tianjin Huanhu Hospital from November 2020 to May 2022 were studied.The pathological results showed 132 cases of benign thyroid nodules and 68 cases of malignant thyroid nodules.All patients were diagnosed by high frequency ultrasound and color Doppler ultrasound.The sonographic features and blood flow parameters[carotid artery peak systolic velocity(PSV)and vascular resistance index(RI)]were analyzed.The diagnostic efficacy of high frequency ultrasound,color Doppler ultrasound and combined diagnosis for benign and malignant thyroid nodules was compared.Results There were significant differences in the characteristics of high-frequency ultrasonography between benign and malignant thyroid nodules(nodule structure,nodule echo,nodule morphology,nodule edge,nodule calcification)(P<0.05).There was significant difference in color Doppler flow characteristics between benign and malignant thyroid nodules(PSV、RI)(P<0.05).The diagnostic accuracy,sensitivity and specificity of combined diagnosis of benign and malignant thyroid nodules were higher than those of high frequency ultrasound and color Doppler ultrasound,and the difference was statistically significant(P<0.05).Conclusion High frequency ultrasound combined with color Doppler ultrasound has high accuracy,sensitivity and specificity in the diagnosis of benign and malignant thyroid nodules,and its diagnostic value is higher than that of single ultrasound detection.
