3D打印乳腺托架增加乳腺癌调强放疗摆位准确度研究 被引量:3

摘要 目的探讨3D打印乳腺托架和普通头枕两种固定方式在乳腺癌调强放疗中摆位准确度。从摆位误差分析3D打印乳腺托架和普通头枕两种固定装置的效果。方法选取四川省人民医院2022年3月至8月接受调强放疗的80例乳腺癌患者,随机分为两组,3D乳腺托架组和普通头枕组各40例。两组患者每周都进行锥体束CT(CBCT)扫描,用热塑膜固定胸部和下颌,将验证图像和计划图像配准,测量治疗中心坐标误差。用SPSS 27.0统计软件分析两组患者在乳腺/胸壁部位、锁骨上下部位、腋窝部位的左右(Lat)、头脚(Lng)以及腹背(Vrt)3个方向的摆位误差。结果两组患者基本资料差异无统计学意义。3D乳腺托架组在乳腺/胸壁部位的Lat、Lng、Vrt 3个方向摆位误差分别是(1.79±1.52)mm、(1.91±1.67)mm、(1.89±1.27)mm;锁骨上下部位:(1.72±1.38)mm、(1.95±1.53)mm、(1.68±1.31)mm;腋窝部位:(1.92±1.31)mm、(2.21±1.66)mm、(1.85±1.38)mm;普通头枕组在乳腺/胸壁部位的Lat、Lng、Vrt 3个方向摆位误差分别是(2.71±1.89)mm、(2.97±2.14)mm、(2.95±2.31)mm;锁骨上下部位:(2.92±2.00)mm、(2.90±1.66)mm,(2.86±1.61)mm;腋窝部位:(3.10±1.74)mm、(3.19±1.72)mm、(3.14±1.81)mm。3D乳腺托架组准确度高,误差小于普通头枕组,两组摆位误差比较差异有统计学意义(P<0.05)。结论3D打印乳腺托架固定摆位准确度更高,3个测量位置的摆位误差均小于普通头枕固定。 Objective To explore the positioning accuracy of 3D printed breast bracket and common headrest in intensity modulated radiotherapy(IMRT)for breast cancer.To analyze the data of 3D printed breast bracket and common headrest from the positioning error,and study the effect of two kinds of fixing devices.Methods Eighty patients with breast cancer who were received IMRT in Sichuan Provincial People s Hospital from March to August 2022 were selected.They were divided into two groups:3D breast bracket group(40 cases)and common headrest group(40 cases).There was no statistical difference between the two groups in basic data.Both groups of patients underwent cone beam CT scanning every week,and the chest and mandible were fixed with thermoplastic film.The verification image was registered with the planned image,and the treatment center coordinate error was measured.The positioning errors of breast/chest wall,supraclavicular and inferior,armpit in the left and right(Lat),head and foot(Lng),abdomen and back directions(Vrt)were analyzed by SPSS 27.0 statistical software.Results In the 3D breast bracket group,the positioning errors of Lat,Lng and Vrt in the breast/chest wall were(1.79±1.52)mm,(1.91±1.67)mm,(1.89±1.27)mm,supraclavicular and inferior(1.72±1.38)mm,(1.95±1.53)mm,(1.68±1.31)mm,armpit(1.92±1.31)mm,(2.21±1.66)mm,and(1.85±1.38)mm,respectively;In the common headrest group,the positioning errors of Lat,Lng and Vrt in the breast/chest wall were(2.71±1.89)mm,(2.97±2.14)mm,(2.95±2.31)mm,supraclavicular and inferior(2.92±2.00)mm,(2.90±1.66)mm,(2.86±1.61)mm,armpit(3.10±1.74)mm,(3.19±1.72)mm and(3.14±1.81)mm respectively.The accuracy of the 3D breast bracket group was high,and the error was smaller than that of the common headrest group.The comparison of the positioning errors between the two groups was statistically significant P<0.05.Conclusion The 3D printed breast bracket has high positioning accuracy,and the positioning error is less than that of common headrest fixation at three measuring positions.
