摘要
目的探究宫颈上皮内瘤样病变(CIN)程度及人乳头瘤病毒(HPV)感染与宫颈组织肿瘤转移相关因子-1(MTA1)、B淋巴细胞瘤-2基因(Bcl-2)表达的相关性。方法选取2018年1月-2021年3月于海南西部中心医院妇科行各项检查的107例CIN患者为病例组,并根据CIN病变程度分为鳞癌组(宫颈鳞癌)31例、高级别病变组(CINⅡ~Ⅲ)27例、低级别病变组(CINⅠ)49例,30例慢性宫颈炎患者为对照组,分析宫颈组织MTA1、Bcl-2水平与CIN病变程度及HPV感染的关系。结果病例组与对照组患者首次性生活年龄、孕次、避孕方式、性伴侣数比较差异有统计学意义(P<0.05);病例组MTA1和Bcl-2阳性率高于对照组(P<0.05),且随着病变程度加重阳性率升高(P<0.05);病例组高危HPV感染患者宫颈组织MTA1、Bcl-2阳性率显著高于低危HPV感染患者(P<0.05)。结论HPV感染与CIN患者宫颈癌病变相关,其可能通过影响组织MTA1、Bcl-2上调表达促进癌变进展,临床还需进一步验证。
OBJECTIVE To explore the correlation between degree of cervical intraepithelial neoplasia(CIN),human papillomavirus(HPV)infection and expressions of metastasis-associated gene 1(MTA1)and B-cell lymphoma-2(Bcl-2)in cervical tissues.METHODS Totally 107 CIN patients who received gynecological examinations in Western Hainan Central Hospital from Jan 2018 to Mar 2021 were assigned as the case group and were divided into the squamous cell carcinoma group(cervical squamous cell carcinoma)with 31 cases,the high-grade lesion group(CINⅡ-Ⅲ)with 27 cases and the low-grade lesion group(CIN I)with 49 cases according to the degree of CIN lesions.Totally 30 patients who had chronic cervicitis were chosen as the control group.The relationship between the levels of cervical tissue MTA1,Bcl-2 and the degree of CIN lesion and HPV infection was observed.RESULTS There were significant differences in the age of initial sexual intercourse,gravidity,contraceptive method and number of sexual partners between the case group and the control group(P<0.05).The positive rates of MTA1 and Bcl-2 of the case group were higher than those of the control group(P<0.05),and the positive rates were increased with the exacerbation of lesions(P<0.05).In the case group,the positive rates of MTA1 and Bcl-2 of the patients with high-risk HPV infection were remarkably higher than those of the patients with low-risk HPV infection(P<0.05).CONCLUSION The HPV infection is associated with the cervical lesions of the CIN patients,which may promote the progression of cancer by affecting the upregulations of MTA1 and Bcl-2 in cervical tissues,and this needs further clinical verification.
