摘要
目的探讨膀胱癌术后外周血白细胞介素-8(IL-8)/白细胞介素-8受体1(CXCR1)/白细胞介素-8受体2(CXCR2)轴信号通路基因表达水平及其与尿路感染(UTI)的关系。方法选取2018年12月-2021年12月在中国人民解放军联勤保障部队第九二八医院行腹腔镜膀胱癌根治术后发生UTI的80例患者为感染组,同期80例行腹腔镜膀胱癌根治术后未发生UTI的患者为对照组。分析UTI病原菌分布,荧光定量聚合酶链式反应(PCR)法检测两组患者术后外周血IL-8、CXCR1、CXCR2 mRNA相对表达量,比较两组患者外周血IL-1β、肿瘤坏死因子-α(TNF-α)、前列腺素E2(PGE2)、基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)、成纤维细胞生长因子(FGF)水平。结果80例UTI患者共分离82株病原菌,以革兰阴性菌(74.39%)为主;感染组IL-8、CXCR1、CXCR2 mRNA相对表达量均高于对照组(P<0.05),TNF-α、IL-1β、PGE2、MMP-9、VEGF、FGF水平均高于对照组(P<0.05);感染组患者IL-8、CXCR1、CXCR2表达分别与TNF-α、IL-1β、PGE2、MMP-9、VEGF、FGF呈正相关(P<0.05)。结论腹腔镜膀胱癌根治术后UTI与IL-8/CXCR1/2轴信号通路高表达密切相关,且可能会促进患者肿瘤病情发展。
OBJECTIVE To explore the relationship between expression levels of interleukin-8(IL-8)/CXC chemokine receptor 1(CXCR1)/CXC chemokine receptor 2(CXCR2)signaling pathway genes in peripheral blood and postoperative urinary tract infection(UTI)in bladder cancer patients.METHODS From Dec 2018 to Dec 2021,totally 80 bladder cancer patients who underwent laparoscopic radical resection and had postoperative UTI in No.28 Hospital of the PLA Joint Logistics Support Force were assigned as the infection group,meanwhile,80 bladder cancer patients who underwent laparoscopic radical resection but did not have postoperative UTI were chosen as the control group.The distribution of pathogens causing UTI was analyzed,the relative expression levels of postoperative peripheral blood IL-8,CXCR1 and CXCR2 mRNA were detected by fluorescent quantitative polymerase chain reaction(PCR).The levels of peripheral blood IL-1β,tumor necrosis factor-α(TNF-α),prostaglandin E2(PGE2),matrix metalloproteinase-9(MMP-9),vascular endothelial growth factor(VEGF)and fibroblast growth factor(FGF)were compared between the two groups of patients.RESULTS Totally 82 strains of pathogens were isolated from the 80 patients with UTI,74.39%of which were gram-negative bacteria.The relative expression levels of IL-8,CXCR1 and CXCR2 mRNA of the infection group were higher than those of the control group(P<0.05);the levels of TNF-α,IL-1β,PGE2,MMP-9,VEGF and FGF of the infection group were higher than those of the control group(P<0.05).The expressions of IL-8,CXCR1 and CXCR2 of the infection group respectively show positive correlation with TNF-α,IL-1β,PGE2,MMP-9,VEGF and FGF(P<0.05).CONCLUSION The postoperative UTI in the bladder cancer patients undergoing laparoscopic radical resection is closely associated with the high expression of IL-8/CXCR1/2 signaling pathway genes,which may promote the development of illness condition.
