摘要
目的研究腹腔镜下小儿疝高位结扎术治疗小儿疝气的临床作用。方法方便选择2020年1月—2021年12月在邳州市中医院接受治疗的78例小儿疝气患儿作为研究对象,随机数表法分为两组,采用传统开腹手术治疗的20例纳入对照组,采用腹腔镜下小儿疝高位结扎术治疗的58例纳入观察组。对比两组患儿治疗效果、围术期指标、手术并发症发生率及术后6个月复发率。结果观察组治疗总有效率为100.00%,高于对照组的70.00%,差异有统计学意义(χ^(2)=14.862,P<0.05)。观察组术中出血量、手术切口大小、手术时间、术后下床时间、排气时间、持续疼痛时间、住院时间分别为(1.50±0.32)mL、(0.83±0.09)cm、(12.26±2.37)min、(2.16±0.61)d、(24.19±3.64)h、(23.45±5.32)h、(2.78±1.02)d,均优于对照组,差异有统计学意义(t=17.837、20.831、12.880、17.541、10.815、7.375、17.315,P<0.05)。观察组并发症发生率为5.17%,低于对照组的35.00%,差异有统计学意义(χ^(2)=9.320,P<0.05)。观察组术后6个月复发率为0.00%,低于对照组的20.00%,差异有统计学意义(χ^(2)=8.462,P<0.05)。结论予以小儿疝气患儿腹腔镜下小儿疝高位结扎术治疗效果显著,患儿术后恢复效果好,且并发症少,半年内复发率低。
Objective To study the clinical effect of laparoscopic pediatric hernia high ligation in the treatment of pediatric hernia.Methods 78 pediatric hernia patients who received treatment at Pizhou Traditional Chinese Medicine Hospital from January 2020 to December 2021 were conveniently selected as the research subjects and randomly divided into two groups using a number table method.20 cases treated with traditional open surgery were included in the control group,and 58 cases treated with laparoscopic pediatric hernia high ligation were included in the observation group.The treatment effect,perioperative indicators,incidence of surgical complications and recurrence rate at 6 months after surgery were compared between the two groups of children.Results The total effective rate of treatment in the observation group was 100.00%,higher than 70.00% in the control group,the difference was statistically significant(χ^(2)=14.862,P<0.05).The intraoperative bleeding,surgical incision size,operative time,postoperative bed time,time to exhaustion,duration of pain,and hospital stay in the observation group were(1.50±0.32)mL,(0.83±0.09)cm,(12.26±2.37)min,(2.16±0.61)d,(24.19±3.64)h,(23.45±5.32)h,(2.78±1.02)d,were better than those in the control group,the difference was statistically significant(t=17.837,20.831,12.880,17.541,10.815,7.375,17.315,P<0.05).The incidence of complications in the observation group was 5.17%,lower than the 35.00% in the control group,the difference was statistically significant(χ^(2)=9.320,P<0.05).The recurrence rate at 6 months after surgery in the observation group was 0.00%,lower than the 20.00% in the control group,the difference was statistically significant(χ^(2)=8.462,P<0.05).Conclusion The treatment of children with pediatric hernia with laparoscopic pediatric hernia high ligation was effective,and the children recovered well after surgery,and the complications are few,and the recurrence rate is low within six months.
