摘要
目的分析苦劳汤灌洗与加味苦参汤坐浴联合在肛周脓肿治疗中的效果。方法方便选取2018年1月—2021年1月江苏省东台市中医院肛肠科肛周脓肿患者120例为研究对象,以随机数表法分为观察组和对照组,各60例。对照组予以负压封闭引流术,观察组在对照组基础上实施苦劳汤灌洗与加味苦参汤坐浴联合治疗,对比两组患者治疗结果。结果观察组总有效率(96.67%)比对照组(85.00%)更高,差异有统计学意义(χ^(2)=4.904,P<0.05)。治疗后,观察组的肛门失禁评分、肛管静息压均低于对照组,肛管最大收缩压高于对照组,差异有统计学意义(P<0.05)。治疗后,观察组的IL-6、IL-8、TNF-α水平均低于对照组,差异有统计学意义(P<0.05)。观察组的肉芽生成时间、肛周愈合时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。结论肛周脓肿患者接受苦劳汤灌洗与加味苦参汤坐浴联合治疗,可提高治疗的效果,促进患者肛门功能、炎性因子指标水平的改善,且可缩短康复的时间。
Objective To analyze the effect of Kulao decoction lavage combined with flavoured Kuh-seng decoction sitting bath in the treatment of perianal abscess.Methods 120 patients with perianal abscess in the department of proctology of Dongtai Hospital of Traditional Chinese Medicine were conveniently selected as the research objects from January 2018 to January 2021,according to the random number table,they were divided into control group and observation group,60 cases in each group.The control group was treated with negative pressure closed drainage,the observation group was treated with Kulao Tang irrigation and modified Kushen Tang sitz bath on the basis of the con⁃trol group.The treatment results of the two groups were analyzed.Results The total effective rate of the observation group(96.67%)was higher than that of the control group(85.00%),the difference was statistically significant(χ^(2)=4.904,P<0.05).After treatment,the anal incontinence score and anal resting pressure in the observation group were lower than those in the control group,and the maximum anal systolic pressure was higher than that in the control group,the difference was statistically significant(P<0.05).After treatment,the levels of IL-6,IL-8,TNF-αin the ob⁃servation group the levels were lower than those in the control group,the difference was statistically significant(P<0.05).The granulation formation time,perianal healing time,and hospitalization time of the observation group were shorter than those of the control group,the difference was statistically significant(P<0.05).Conclusion For patients with perianal abscess,the combination of Kulao decoction lavage combined with flavoured Kuh-seng decoction sitting bath can improve the therapeutic effect,promote the improvement of the patients'anal function and inflammatory fac⁃tor index level,and shorten the time of recovery.
