精准定位下神经内镜与显微镜治疗基底节脑出血疗效比较 被引量:2

Comparison of the Effect of the Neuroendoscopy and Microscope of Precise Localization in the Treatment of Basilar Ganglia Hemorrhage

摘要 目的比较精准定位下神经内镜与开颅显微镜治疗基底节脑出血的临床效果。方法61例脑出血患者按随机数字表法分为神经内镜手术组30例和开颅显微镜手术组31例。比较2组患者血肿清除率、手术时间、术中出血量、格拉斯哥预后评分(Glasgow Outcome Scale,GOS)、日常生活活动评分(activities of daily living score,ADL)及术后并发症发生情况。结果神经内镜手术组患者血肿清除率、术中出血量、手术时间及GOS评分、ADL评分均优于开颅显微镜组(P<0.05);神经内镜手术组与开颅显微镜手术组术后再出血、颅内感染、消化道溃疡发生情况差异无统计学意义(P>0.05),开颅显微镜手术组肺部感染高于神经内镜手术组(P<0.05)。结论在治疗基底节脑出血时,神经内镜手术组较开颅显微镜手术组能更高效地清除血肿,且术中出血量小,对神经损伤小,临床疗效更佳。 Objective To compare the clinical efficacy of the neuroendoscopy and craniotomy microscope of precise localization in the treatment of basal ganglia hemorrhage.Methods 61 patients with cerebral hemorrhage were selected and randomly assigned to the neuroendoscopic surgery group(30 cases)and the craniotomy microscope surgery group(31 cases)by a random number table method.Hematoma clearance rate,operation time,intraoperative blood loss,Glasgow Outcome Scale(GOS),Activities of daily living score(ADL)and postoperative complications were compared between the two groups.Results Hematoma clearance rate,intraoperative blood loss,operation time,GOS score and ADL score of the neuroendoscopy surgery group were better than those of the craniotomy microscope group(P<0.05).There was no significant difference in the incidence of postoperative rebleeding,intracranial infection,and digestive tract ulcer between the neuroendoscopic surgery group and the craniotomy microsurgery group(P>0.05).The pulmonary infection in the craniotomy microsurgery group was higher than that in the neuroendoscopic surgery group(P<0.05).Conclusion In the treatment of basal ganglia hemorrhage,the neuroendoscopic surgery group is more efficient in clearing hematoma compared to the craniotomy microscope surgery group,and the intraoperative bleeding volume is small,with less nerve damage and better clinical efficacy.
