摘要
目的探究白噪声疗法结合袋鼠式干预对低出生体重儿生长发育的影响。方法选取2020年1月至2021年10月杭州市妇产科医院新生儿重症监护病房(neonatal intensive care unit,NICU)收治的94例低出生体重儿为研究对象,采用随机数字表法分为对照组(n=47)和联合组(n=47),对照组给予常规低出生体重儿护理,联合组在对照组的基础上给予白噪声疗法结合袋鼠式护理,比较两组临床指标、体格发育、智力发育指数(mental development index,MDI)、心理运动发育指数(psychomotor development index,PDI)及神经行为发育[新生儿神经行为测定(neonatal behavioral neurological assessment,NBNA)]情况。结果干预后,联合组患儿喂养耐受天数、恢复出生体重时间、住院天数均显著低于对照组(P<0.05);联合组新生儿头围、体重、身长增长均显著高于对照组(P<0.05);干预后,两组新生儿MDI、PDI、NBNA评分均显著升高(P<0.05),且上述指标联合组显著高于对照组(P<0.05)。结论白噪声疗法结合袋鼠式干预用于低出生体重儿的临床护理,可有效促进患儿智力、体格及神经行为的发育。
Objective To explore the effects of white noise therapy combined with kangaroo intervention on the growth and development of low birth weight neonates.Methods A total of 94 cases of low birth weight infants who were admitted to department of neonatal intensive care unit(NICU)of Hangzhou Obstetrics and Gynecology Hospital from January 2020 to October 2021 were selected and divided into the control group(n=47)and the combined group(n=47)according to random number table method.The control group received routine nursing intervention for low birth weight neonates,while the combined group was given white noise therapy combined with kangaroo intervention nursing.The clinical indicators,physical development,mental development mental development index(MDI),psychomotor development index(PDI)and neurobehavioral development[neonatal behavioral neurological assessment(NBNA)]were compared between the two groups.Results After intervention,the feeding tolerance days,the time to return to birth weight and hospital stay in the combined group were significantly shorter than those in the control group(P<0.05).The growths of head circumference,body mass and body length were significantly higher in the combined group than those in the control group(P<0.05).After intervention,the scores of MDI,PDI and NBNA in the two groups of neonates were significantly increased(P<0.05),and the above scores in the combined group were significantly higher compared to the control group(P<0.05).Conclusion White noise therapy combined with kangaroo intervention in the clinical nursing of low birth weight neonates can effectively promote the mental,physical and neurobehavioral developments of children,and it is worthy of recommendation.
