摘要
目的分析早期完全性水泡状胎块的病理诊断结果。方法选取本院于2018年1月至2023年1月收治的早期完全性水泡状胎块患者48例,观察早期完全性水泡状胎块患者的病理检查结果、免疫组化检查结果、年龄分布情况、血β-HCG浓度检查结果。结果病理检查结果如下:绒毛大小肉眼可见,直径0.3~1 cm,平均直径为(1.03±0.46)cm,肿块填满宫腔。48例患者血β-HCG浓度范围为7.854×10^(3)~3.527×10^(6) IU/L,血β-HCG浓度平均数为3.115×10^(5) IU/L,血β-HCG浓度中位数为8.40×10^(4) IU/L,血β-HCG浓度>1.0×10^(5) IU/L患者共29例,占比60.42%。结论大体检查下发现绒毛、肿块、水泡,光镜下发现指状的绒毛伴有绒毛间质细胞增生及环周滋养细胞花边样增生、中央水池形成情况,则可以怀疑患者存在早期完全性水泡状胎块。通过免疫组化Ki-67、p53、p57的表达检查结果以及患者发病年龄、血β-HCG浓度可以对早期完全性水泡状胎块进行鉴别诊断,最后通过早期完全性水泡状胎块病理分析明确诊断。
Objective To study the pathological diagnosis of early complete vesicular mole.Methods Forty-eight patients with early complete vesicular moles admitted to our hospital from January 2018 to January 2023 were selected to observe the pathological examination results,immunohistochemical examination results,age distribution,blood β-HCG concentration check results.Results The results of pathological examination were as follows:the size of villi was visible to the naked eye,the diameter was 0.3-1 cm,the average diameter was(1.03±0.46)cm,and the mass filled the uterine cavity.Blood of 48 patients β-HCG concentration range is 7.854×10^(3)-3.527×10^(6) IU/L,blood β-HCG concentration is 3.115×10^(5) IU/L,blood β-HCG concentration is 8.40×10^(4) IU/L,blood β-HCG concentration>1.0×10^(5) IU/L patients were 29,accounting for 60.42%.Conclusion If villi,masses and blisters are found under gross examination,and finger-like villi are found under light microscope with hyperplasia of villous stromal cells,peritrophoblastic lace-like hyperplasia and central pool formation,it can be suspected that the patient has early complete vesicular mole.According to the expression of Ki-67,p53 and p53 in immunohistochemistry,the age of onset and blood β-HCG concentration can be used for differential diagnosis of early complete vesicular moles,so that the diagnosis accuracy can be achieved through pathological analysis of early complete vesicular moles.
