摘要
目的分析通痹胶囊联合托法替布治疗老年类风湿性关节炎(rheumatoid arthritis,RA)患者的临床效果,并探讨其对血清学指标、免疫指标等的影响。方法选取2021年1月至2022年1月信阳市息县人民医院收治的80例老年RA患者作为研究对象,按随机数字表分为两组。对照组(n=40)采用常规西药治疗,观察组(n=40)采用通痹胶囊联合常规西药治疗。比较两组患者治疗前后的血清学指标变化、不良反应、临床疗效等。结果疗程结束后,观察组晨僵、畏寒、关节屈伸不利等中医证候积分均低于对照组,差异有统计学意义(P<0.05);观察组的血清学指标C-反应蛋白(CRP)、粒细胞-巨噬细胞集落刺激因子(GM-CSF)、B细胞趋化因子CXC配体13(CXCL13)水平均低于对照组,差异有统计学意义(P<0.05);观察组治疗总有效率为95.00%,对照组为80.00%,差异有统计学意义(P<0.05);治疗期间,两组的药物不良反应发生率差异无统计学意义(P>0.05)。结论老年RA患者采用通痹胶囊与托法替布联合治疗,能够有效缓解患者症状,改善血清学指标水平,且不增加药物不良反应,安全性高,取得显著的治疗效果,进一步控制病情发展。
Objective To analyze the clinical efficacy of Tongbi capsule combined with Tofacitinib in the treatment of senile patients with rheumatoid arthritis(RA),and to explore its impact on serological and immune indicators.Methods From January 2021 to January 2022,80 elderly RA patients admitted to our hospital in Xixian County People's Hospital of Xinyang City were selected as the study subjects,and were divided into two groups according to a random number table.The control group(n=40)was treated with conventional western medicine,while the observation group(n=40)was treated with Tongbi capsule combined with conventional western medicine.The changes in serological indicators,adverse reactions,and clinical efficacy of the two groups of patients before and after treatment were compared.Results After the course of treatment,the scores of traditional Chinese medicine syndromes such as morning stiffness,chills,and poor joint flexion and extension in the observation group were lower than those in the control group,with a statistically significant difference(P<0.05);The serum levels of C-reactive protein(CRP),granulocyte macrophage colony stimulating factor(GM-CSF),and B cell chemokine CXC ligand 13(CXCL13)in the observation group were significantly lower than those in the control group,with a statistically significant difference(P<0.05);The total effective rate was 95.00%in the observation group and 80.00%in the control group,with a statistically significant difference(P<0.05);However,during the treatment period,there was no statistically significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion The treatment of elderly RA with Tongbi capsule combined with Tofacitinib can effectively alleviate symptoms,improve the level of serological indicators,without increasing adverse drug reactions,and has high safety.It has achieved significant therapeutic effects and further controlled the development of the disease.
