摘要
目的 探讨基于营养分析的个体化营养指导对妊娠期糖尿病孕妇妊娠结局的影响。方法 选取2020年6月-2022年3月在本院产科门诊产检并确诊为妊娠期糖尿病(GDM)孕妇200例为研究对象,随机分为观察组100例与对照组100例。对照组给予常规孕期健康教育和营养指导,观察组在对照组基础上根据GDM确诊时体成分分析结果制定个体化营养指导,并于孕32周和孕36周再次检测体成分并对营养方案进行调整。比较两组患者干预后不同时点空腹血糖、餐后2h血糖水平及不良妊娠结局发生率。结果 干预后6次血糖水平检测中,观察组FPG、2hPBG均较对照组低(P<0.05);观察组妊娠期高血压疾病发生率差异较对照组低(P=0.030),但在羊水过多、早产、产后出血、巨大儿、新生儿低血糖、剖宫产和低出生体重儿上两组孕妇发生率差异无统计学意义(P>0.05)。结论 基于营养分析的个体化营养指导可有效改善妊娠期间GDM患者的血糖水平,降低不良妊娠结局的发生风险。
Objective To investigate the effect of individualized nutritional guidance based on nutritional analysis on pregnancy outcomes of pregnant women with gestational diabetes mellitus(GDM).Methods A total of 200 pregnant women who were diagnosed with gestational diabetes mellitus(GDM)in the obstetric outpatient department of our hospital during the obstetric examination from June 2020 to March 2022 were enrolled,and they were randomly divided into an observation group of 100 cases and a control group of 100 cases.The control group was given routine pregnancy health education and nutrition guidance,and the observation group was given personalized nutrition guidance based on the results of body composition analysis at the time of GDM diagnosis on the basis of the control group.The levels of fasting blood glucose(FPG),2h postprandial blood glucose(2h PBG)and the incidence of adverse pregnancy outcomes were compared between the two groups after intervention.Results After intervention,FPG and 2hPBG in the observation group were lower than those in the control group(P<0.05)in all six point time.The difference in the incidence of hypertensive disorder complicating pregnancy in the observation group was lower than that in the control group(P=0.030).There was no significant difference in the incidence rates of preterm birth,postpartum hemorrhage,macrosomia,neonatal hypoglycemia,cesarean section and low birth weight infants between the two groups(P>0.05).Conclusion Individualized nutritional guidance based on nutritional analysis can effectively improve blood glucose levels in GDM patients during pregnancy and reduce the risk of adverse pregnancy outcomes.
