摘要
目的 总结与分析宫颈上皮内瘤变(cervical intraepithelial neoplasis,CIN)筛查中高危型人乳头瘤病毒(high-risk human papillomavirus,HR-HPV)、宫颈液基薄层细胞检测(thinprep cytologic test,TCT)应用效果和临床价值。方法 将2021年1~12月在本院进行宫颈筛查的1000例女性患者作为本次研究的对象,主要利用第三代杂交捕获定量分型(DH3)对患者展开14种HPV分型检测,并给予TCT检测,以病理活检作为金标准,对检查结果进行对比分析。结果 经病理活检检查1000例患者中184例确诊阳性,816例为阴性,其中HPV阳性241例,阴性759例,TCT阳性228例,阴性772例,HPV+TCT阳性196例;HPV灵敏度(77.72%)、特异度(87.99%)以及阳性预测值(59.34%)和阴性预测值(94.60%)均其低于TCT和TCT+HPV检查,而HPV+TCT灵敏度(97.83%)、特异度(98.04%)、阳性预测值(91.84%)、阴性预测值(99.50%)均高于HPV和TCT(84.24%、91.04%、67.98%、96.24%)单项检查。结论 TCT和HPV联合检测较其单项检测特异度、灵敏度更高,更具有临床价值,故值得临床广泛推广用于宫颈上皮内瘤变筛查中。
Objective To analyze the clinical value of high-risk human papillomavirus(HR-HPV)and thinprep cytologic test(TCT)on screening of cervical intraepithelial neoplasia(CIN).Methods 1000 female patients undergoing the cervical screening(2021.01-2021.12)were chosen as research subjects.14 HPV types were measured by daltonbio hybrid capture 3(DH3)and then TCT.The pathological biopsy results were seen as gold standard.The examination outcomes were probed.Results 184 positive cases and 816 negative cases were confirmed by the pathological biopsy.241 positive cases and 759 negative cases were defined by HPV;228 positive cases and 772 negative cases were defined by TCT;196 positive cases were defined by HPV+TCT.The diagnosis sensitivity,specificity,positive predictive value and negative predictive value based on HPV were 77.72%,87.99%,59.34%and 94.60%,which were significantly lower than those based on TCT or TCT+HPV.The diagnosis sensitivity,specificity,positive predictive value and negative predictive value based on HPV+TCT were 97.83%,98.04%,91.84%and 99.50%,which were significantly higher than those based on HPV or TCT(84.24%,91.04%,67.98%,96.24%)(P<0.05).Conclusion The combination of TCT and HPV can increase the diagnosis specificity and sensitivity of cervical intraepithelial neoplasia.It shall be widely applied in clinical examination.
