摘要
目的 探讨经肌间隙入路复位固定术治疗胸腰椎骨折的效果及对腰椎功能的影响。方法 选取2018年1月-2022年1月本院收治的100例胸腰椎骨折患者为研究对象,按掷硬币法随机分为参照组与研究组,各50例。其中参照组应用经后路复位固定椎间融合术治疗,研究组应用经肌间隙入路复位固定术治疗,比较分析两组患者手术相关情况、手术前后疼痛程度、腰椎功能及术后并发症发生情况。结果 与参照组相比,研究组手术时间、术中出血量、术后下床时间、术后引流量及住院时间均明显更低(P<0.05)。两组术后VAS评分、RDQ评分、椎体前缘高度较术前均有变化,且与参照组相比,研究组RDQ评分更低,椎体前缘高度明显更高(P<0.05)。两组术后并发症发生率比较无显著差异(P>0.05)。结论 针对胸腰椎骨折临床开展经肌间隙入路复位固定术能够获得比较理想的治疗效果,有助于促进患者腰椎功能恢复,提高生活质量,值得临床应用。
Objective To investigate the effect of reduction and fixation via intramuscular approach on thoracolumbar fracture and its influence on lumbar function.Methods 100 patients with thoracolumbar fracture who were treated from January 2018 to January 2022 were selected as the study subjects.They were randomly divided into the reference group and the study group by coin tossing method,with 50 patients each.The reference group was treated with posterior reduction and fixation and interbody fusion,and the study group was treated with intramuscular reduction and fixation.The related conditions of the two groups,the degree of pain before and after the operation,the lumbar function and the occurrence of postoperative complications were compared and analyzed.Results Compared with the reference group,the operation time,intraoperative blood loss,postoperative bed time,postoperative drainage volume and hospital stay in the study group were significantly lower(P<0.05).The VAS score,rdq score and the height of the anterior edge of the vertebral body in the two groups after operation were changed compared with those before operation.Compared with the reference group,the rdq score of the study group was lower and the height of the anterior edge of the vertebral body was significantly higher(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion The intramuscular reduction and fixation for thoracolumbar fracture can obtain ideal therapeutic effect,which is helpful to promote the recovery of lumbar function and improve the quality of life,and is worthy of clinical application.
